集束化护理在急性缺血性脑卒中患者中的应用效果

摘　　要：目的探究集束化护理在急性缺血性脑卒中(AIS)患者中的应用效果。方法选择2020年10月—2021年10月于本院卒中中心治疗的140例AIS患者作为研究对象,采取随机数表法将其分为对照组和实验组两组,每组各70例。对照组实施常规护理,实验组在此基础上实施集束化护理。采用美国国立卫生院卒中量表(NIHSS)、Barthel指数评定量表、脑卒中专用生活质量量表(SS-QOL)评估两组患者护理干预前后的神经功能缺损、日常生活能力、生活质量,调查消化道出血、皮肤、牙龈出血、皮下淤斑、泌尿系统出血、颅内出血等并发症发生情况及护理满意率。结果干预前,两组患者的NIHSS评分、Barthel评分、SS-QOL评分比较,差异不具有统计学意义(P>0.05);干预后,实验组患者的NIHSS评分为(12.35±2.16)分,对照组为(16.68±2.59)分;实验组患者的Barthel评分为(56.26±8.20)分,对照组为(48.79±6.35)分;实验组患者的SS-QOL评分为(166.80±6.62)分,对照组为(145.19±6.80)分;实验组并发症发生率为10.00%,对照组为25.71%;实验组患者对护理的满意度为94.29%,对照组为78.57%。以上所有差异均有统计学意义(P<0.05)。结论集束化护理可改善ASI患者的神经功能,提高患者日常生活能力和生活质量,减少并发症的发生,提高护理满意率。Objective To explore the application effect of cluster nursing in patients with acute ischemic stroke(AIS).Methods 140 patients with acute ischemic stroke who were treated in the stroke center of our hospital from October 2020 to October 2021 were selected as the research objects.The 140 subjects were divided into control group(n=70)and experimental group(n=70)according to random number table method.The patients in the control group were given routine care,and the patients in the experimental group were given cluster care on this basis.The neurological deficit score,daily living ability score,and quality of life score were evaluated by using National Institutes of Health Stroke Scale(NIHSS),Barthel Index rating scale,Stroke specific quality of life scale(SS-QOL).The incidence of gastrointestinal bleeding,skin and gingival bleeding,subcutaneous ecchymosis,urinary system bleeding,intracranial hemorrhage and the nursing satisfaction rate were compared between the two groups.Results Before the intervention,there were no significant difference in the scores of NIHSS,Barthel,and SS-QOL between the two groups(P>0.05).After the intervention,the NIHSS score of the experimental group was(12.35±2.16),and that of the control group was(16.68±2.59);The Barthel score was(56.26±8.20)in the experimental group and(48.79±6.35)in the control group;The SS-QOL score was(166.80±6.62)in the experimental group and(145.19±6.80)in the control group.The incidence of complications in the experimental group was 10%,the control group was 25.71%;the satisfaction rate of the patients in the experimental group was 94.29%,and that in the control group was 78.57%.All the above differences were statistically significant(P<0.05).Conclusions The implementation of cluster nursing in ASI patients could improve the neurological function of patients,improve ability of daily living and quality of life of patients,reduce the occurrence of complications and improve nursing satisfaction rate.
