摘　　要：目的 探讨输尿管镜下钬激光治疗输尿管上段结石术(PCNL)中不同的体位对手术效果不同的影响。方法 选取采用PCNL治疗的患者80例。采用随机分组的方式将其分为对照组和观察组,45例采用俯卧位作为对照组,35例采用侧卧位作为观察组,两组采用相同的麻醉、手术方法、护理措施。对两组患者手术中的出血情况、手术时间、血压变化情况进行分析比较。结果 两组患者手术均成功,采用侧卧位患者的出血情况、手术时间均小于采用俯卧位患者;侧卧位患者术中出现血压波动情况少于俯卧位患者,差异有统计学意义(P<0.05)。结论 输尿管镜下PCNL术采用侧卧位降低手术风险,有助于手术质量的提高,值得临床的推广和应用。Objective To investigate the effect of different position on the surgical effect in the treatment of upper ureteral calculi(PCNL)with holmium laser under ureteroscope.Methods 80 patients with PCNL were selected.They were randomly divided into the control group and the observation group.45 cases were in prone position as the control group,and 35 cases in the lateral position as the observation group,The two groups adopted the same anesthesia,surgical methods and nursing measures.The bleeding,operation time and blood pressure changes of the two groups were analyzed and compared.Results The operation was successful in both groups.The bleeding and operation time of patients in lateral position were shorter than those in prone position.The fluctuation of blood pressure in patients in lying position was less than that in patients in prone position,and the difference was statistically significant(P<0.05).Conclusion PCNL under ureteroscope can reduce the operation risk and improve the operation quality,which is worthy of clinical popularization and application.
