以质量反馈为导向的一体化管理在尿道下裂患儿围术期护理中的应用效果

摘　　要：目的探讨以质量反馈为导向的一体化管理在尿道下裂患儿围术期护理中的应用效果。方法选取医院2021年7—12月收治的尿道下裂患儿82例为研究对象,按照组间基本特征具有可比性的原则分为对照组和观察组,每组41例。对照组围术期行常规护理模式,观察组围术期行以质量反馈为导向的一体化管理护理模式。比较两组患儿的术后康复情况、疼痛程度、排尿情况及并发症发生情况。结果观察组术后首次排便时间、平均哭闹时间、导尿管拔除时间、住院天数均短于对照组(P<0.05);观察两组患儿术后第6、10、14、38、62小时的Wong-Baker Face疼痛量表(FPS)评分及特殊病人疼痛评估(FLACC)评分,结果显示,两组5个时点各指标均呈逐渐降低的趋势,时间之间差异有统计学意义(P_(时间)<0.05);术后第10、14、38、62小时各指标均以观察组低于对照组,差异有统计学意义(P_(组间)<0.05),并且随观察时间延长,组间差异逐渐增大,组间随时间变化的差异有统计学意义(P_(交互)<0.05);观察组术后的自由尿流率高于对照组(P<0.05),压力流率低于对照组(P<0.05);观察组的总并发症发生率低于对照组(P<0.05)。结论对尿道下裂患儿围术期采用以质量反馈为导向的一体化管理护理模式能促进患儿康复,降低疼痛程度,改善排尿情况,减少并发症发生率。Objective To investigate the application effect of quality feedback-oriented integrated management in the perioperative care of children with suburethral cleft.Methods 82 cases of children with hypospadias admitted to the hospital from July to December 2021 were selected as the study objects.They were divided into a control group and an observation group according to the comparability between groups,with 41 cases in each group.In the control group,conventional care was provided during the perioperative period,in the observation group,an integrated management care model oriented to quality feedback was provided during the perioperative period.The postoperative recovery,pain,urination and complications of the two groups were compared.Results In the observation group,the time to first defecation,the average time of crying,the time to catheter removal and the number of days in hospital were shorter than the control group(P<0.05).The Wong-Baker Face Pain Scale(FPS)scores and The Face,Legs,Activity,Cry,Consolability Behavioral Scale(FLACC)scores were observed at 6,10,14,38 and 62 hours postoperatively in both groups,the difference was statistically significant(P<0.05),and the difference between groups gradually increased as the observation time increased,the difference between groups over time was statistically significant(P<0.05).The total complication rate in the observation group was lower than the control group(P<0.05).Conclusion The application of quality feedback-oriented integrated management care model in the perioperative period for children with hypospadias could promote recovery,reduce pain,improve urination and reduce the incidence of complications.
