锁定钢板内固定治疗复杂性胫骨平台骨折的效果分析

摘　　要：目的:探讨胫骨平台骨折患者行锁定钢板内固定术的治疗效果。方法:选取2019年2月-2021年12月徐州医科大学附属医院收治的胫骨平台骨折患者40例为观察对象。所有患者行锁定钢板内固定手术治疗,分析患者骨折愈合时间、住院时间、手术时间、术中出血量、切口长度、并发症发生情况,膝关节活动情况。结果:患者术后平均骨折愈合时间(2.24±0.20)个月,平均住院时间(12.41±1.10)d,平均手术时间(86.56±10.41)min,平均术中出血量(278.52±50.61)mL,平均切口长度(10.63±1.22)cm。术后关节僵直患者1例,创伤性关节炎患者1例,并发症发生率5.00%。40例患者Ⅳ型有10例、Ⅴ型5例、Ⅵ型4例;术后4~6 d可接受连续被动锻炼,术后14 d患者可在床上行膝关节主动运动。1例Ⅳ型患者由于惧怕疼痛拒绝锻炼,术后3周可接受连续被动锻炼。结论:针对胫骨平台骨折患者进行锁定钢板内固定术,并发症及出血量少,可有效加快骨折愈合。Objective:To investigate the therapeutic effect of internal fixation with locking plates in patients with tibial plateau fractures.Methods:Forty patients with tibial plateau fractures admitted to the Affiliated Hospital of Xuzhou Medical University from February 2019 to December 2021 were selected as observation subjects.All patients were treated with locking plate internal fixation surgery.The patients'fracture healing time,hospitalization time,operation time,intraoperative bleeding,incision length,complication occurrence,and knee joint movement were analyzed.Results:The average postoperative fracture healing time of the patients was(2.24±0.20)months,the average hospitalization time was(12.41±1.10)d,the average operation time was(86.56±10.41)min,the average intraoperative bleeding was(278.52±50.61)mL,and the average incision length was(10.63±1.22)cm.There was one patient with postoperative joint stiffness and one patient with traumatic arthritis,with a complication rate of 5.00%.Forty patients had ten cases of typeⅣ,five cases of typeⅤ,and four cases of typeⅥ.Patients could receive continuous passive exercise from 4 to 6 d postoperatively,and patients could perform active knee exercises in bed 14 d postoperatively.One patient with typeⅣrefused to exercise due to fear of pain,and was able to receive continuous passive exercise 3 weeks after surgery.Conclusion:Locking plate internal fixation was performed in patients with tibial plateau fractures with few complications and bleeding,which can effectively accelerate fracture healing.
