硝酸异山梨酯联合丹参酮注射液治疗AHF的临床疗效

摘　　要：目的 分析硝酸异山梨酯联合丹参酮注射液治疗急性心力衰竭(AHF)的临床疗效。方法 选取2018年1月至2021年8月于首都医科大学附属北京世纪坛医院治疗的138例AHF患者,根据治疗药物的不同分为研究组(n=73,硝酸异山梨酯联合丹参酮注射液)和对照组(n=65,硝酸异山梨酯)。对比两组血流动力学、心功能、炎性介质以及不良反应。结果 治疗后两组患者CVP、PVR、MPAP均出现下降,其中以研究组变化更为明显,差异有统计学意义(P<0.05)。治疗后两组患者LVEDD、LVESD均出现下降,而LVEF出现上升,其中以研究组变化更为明显,差异有统计学意义(P<0.05)。治疗后两组患者TNF-α、IL-6、hs-CRP水平均出现下降,其中以研究组变化更为明显,差异有统计学意义(P<0.05)。研究组不良反应发生率为19.18%,稍高于对照组的12.31%,但两组比较差异无统计学意义(P>0.05)。结论 硝酸异山梨酯联合丹参酮可有效改善AHF患者血流动力学、心功能、炎性因子,且安全性较高,临床可考虑将其推广应用。Objective To analyze the clinical efficacy of isosorbide nitrate combined with tanshinone injection in the treatment of acute heart failure(AHF). Methods 138 patients with AHF treated in Beijing Shijitan Hospital Affiliated to Capital Medical University from January 2018 to August 2021 were selected,and according to different treatment drugs,they were divided into the study group(n=73,isosorbide nitrate combined with tanshinone injection)and the control group(n=65,isosorbide nitrate). Hemodynamics,cardiac function,inflammatory mediators,and adverse reactions were compared between the two groups. Results After treatment,the CVP,PVR and MPAP in the two groups decreased,and the change was more obvious in the study group,and the difference was statistically significant(P<0.05). After treatment,LVEDD and LVESD decreased in the two groups,while LVEF increased,and the change was more obvious in the study group,and the difference was statistically significant(P<0.05). After treatment,the levels of TNF-α,IL-6 and hs-CRP in the two groups were decreased,and the changes in the study group were more obvious,and the difference was statistically significant(P<0.05). The incidence of adverse reactions in the study group was 19.18%,slightly higher than12.31% in the control group,but there was no significant difference between the two groups(P>0.05). Conclusion Isosorbide nitrate combined with tanshinone can effectively improve hemodynamics,cardiac function,and inflammatory factors in patients with AHF,and has high safety. It can be considered for clinical application.
