红花逍遥片治疗桥本甲状腺炎伴结节的临床观察

摘　　要：目的:探究红花逍遥片联合左甲状腺素钠片治疗桥本甲状腺炎伴结节气滞血瘀证患者的临床效果。方法:72例桥本甲状腺炎伴结节气滞血瘀证患者按随机数字表法分为对照组和干预组,每组36例,两组均口服左甲状腺素钠片,干预组在对照组基础上加用红花逍遥片,连续治疗12周。比较治疗前后甲状腺功能(FT_(3)、FT_(4)、TSH)、自身抗体(TPOAb、TGAb)、白细胞介素-6(IL-6)、甲状腺最大结节最长直径变化,计算结节恶性风险积分、中医证候积分与有效率,观察有无不良反应。结果:治疗后两组患者FT_(3)、FT_(4)比较,差异无统计学意义(P>0.05);两组患者TSH、TPOAb、TGAb、IL-6、甲状腺最大结节最长直径、结节恶性风险积分、中医证候积分均较治疗前改善,且干预组改善更明显,差异均有统计学意义(P<0.05);治疗后干预组有效率为88.89%(32/36),明显高于对照组的63.89%(23/36)(P<0.05);两组均未发生相关不良反应。结论:在左甲状腺素钠片基础上联合红花逍遥片对于甲状腺功能无明显影响,可以更有效调节机体免疫,抑制炎症反应,缩小甲状腺结节,减少结节恶变风险,缓解临床症状,安全可靠。Objective:To explore the clinical effect of Honghua Xiaoyao Tablets combined with Levothyroxine Sodium Tablets in the treatment of hashimoto thyroiditis with nodules by qi stagnation and blood stasis syndrome.Methods:A total of 72 patients with Hashimoto thyroiditis with nodular by qi stagnation and blood stasis syndrome were randomly divided into control group and intervention group according to the random number table method,with 36 cases in each group.Both groups were treated with Levothyroxine Sodium Tablets orally.The intervention group was treated with Honghua Xiaoyao Tablets on the basis of the control group.After twelve-week treatment,the changes of thyroid function(FT_(3),FT_(4),TSH),autoantibodies(TPOAb,TGAb),interleukin-6(IL-6)and the longest diameter of the largest thyroid nodule before and after treatment were compared.The risk score of malignant nodules,TCM syndrome score and effective rate were calculated,and adverse reactions were observed.Results:There was no significant difference in FT_(3) and FT_(4) between the two groups after treatment(P>0.05).After treatment,TSH,TPOAb,TGAb,IL-6,the longest diameter of the largest thyroid nodule,the risk score of malignant nodules and the score of TCM syndromes in the two groups were improved compared with those before treatment,and the improvement in the intervention group was more obvious,the differences were statistically significant(P<0.05).After treatment,the effective rate of the intervention group was 88.89%(32/36),which was significantly higher than 63.89%(23/36)of the control group(P<0.05).No related adverse reactions occurred in both groups.Conclusion:The combination of Levothyroxine Sodium Tablets and Honghua Xiaoyao Tablets has no obvious effect on thyroid function.It can more effectively regulate the body's immunity,inhibit the inflammatory response,reduce thyroid nodules,reduce the risk of malignant transformation of nodules,relieve clinical symptoms,and is safe and reliable.
