基于数据挖掘的中医药治疗小儿腺样体肥大遣方用药分析

摘　　要：目的:基于数据挖掘研究中医药治疗小儿腺样体肥大(AH)遣方用药规律。方法:检索中国知网(CNKI)、万方数据库(Wanfang)、维普(VIP)数据库收集国内自2000年1月至2022年1月中医药治疗AH文献所载医案临证处方,获取用药客观数据。运用方剂计量学相关指标、SPSS Statistics 26.0与SPSS Modeler 18.0软件中关联分析、聚类分析等多元统计学方法对数据进行分析。结果:统计临证处方321首,纳入242味中药,常用药35味,功效统计以清热类、解表类、补虚类、化痰止咳平喘类、活血化瘀类为主。四气以温、寒、平为主,五味以苦、辛、甘为主,归经以肺经、胃经、脾经、肝经为主。得到药对关联规则23条;聚类分析药物组合4类。结论:腺样体肥大的治疗方法为初期外感时以解表通窍、清热利咽为主,方用银翘散、麻黄汤加减,桔梗汤和苍耳子散加减,后期益气健脾固肺,兼以化痰散结,行气活血,常用玉屏风散、消瘰丸加减。Objective:To study the patterns of medication used in the treatment of pediatric adenoid hypertrophy(AH)by traditional Chinese medicine(TCM)based on data mining.Methods:The medical cases and prescriptions contained in the Chinese medicine literature database for AH treatment from January 2000 to January 2022 were searched from China Knowledge Network(CNKI),Wanfang database(Wanfang)and VIP database to obtain objective data on the use of medicines.The prescriptions were analyzed by multivariate statistical methods such as correlation analysis and cluster analysis in SPSS Statistics 26.0 and SPSS Modeler 18.0 software.Results:Totally 321 prescriptions of clinical evidence were counted,including 242 Chinese medicines.There are 35 commonly used drugs,and the efficacy are mainly heat-clearing drugs,drugs for relieving exterior syndrome,tonic drugs,resolving phlegm and relieving cough and asthma drugs,and blood-activating and blood-stasis removing drugs.The four characters are mainly warm,cold and flat,and the five flavors are mainly bitter,spicy and sweet.The channel tropism are mainly lung,stomach,spleen and liver meridians.Totally 23 association rules for drug pairs were obtained,and there are clustering analysis of combinations of 4 classes.Conclusion:The treatment of AH is mainly to relieve exterior symptoms and relieve obstruction,clear away heat and relieve sore throat at the initial stage of exogenous diseases,with modified Yinqiao powder(银翘散)and Mahuang decoction(麻黄汤),modified Jugeng decoction and Cangerzi powder(桔梗汤和苍耳子散),and later on,to replenish Qi,strengthen spleen and strengthen lung,and to eliminate phlegm,dissipate stagnation,promote Qi circulation and promote blood circulation.Yupingfeng powder(玉屏风散)and Xiaoluo pill(消瘰丸)are commonly used.
