摘 要:目的 观察血浆热休克蛋白70 (HSP70)、褪黑素(MT)、可溶性细胞黏附因子(sICAM-1)水平与新生儿窒息的相关性。方法 选取2019年3月至2021年3月南阳医学高等专科学校第一附属医院新生儿科收治窒息新生儿为窒息组(74例),另选足月健康新生儿为对照组(67例)。两组新生儿均出生24 h内采取脐动脉血,分离血清检验两组HSP70、MT、sICAM-1水平,比较两组及新生儿窒息不同窒息程度患儿的HSP70、MT、sICAM-1水平,并采用Pearson相关性分析HSP70、MT、sICAM-1与Apgar评分的相关性。结果 窒息组血清中的HSP70、sICAM-1高于对照组,MT低于对照组(P<0.05);74例窒息新生儿中,窒息程度轻度者32例(轻度组),重度者42例(重度组),两组HSP70、sICAM-1水平比较:轻度组<重度组,两组MT水平比较:轻度组>重度组(P<0.05);相关性分析结果显示,HSP70与sICAM-1与窒息程度均呈负相关,MT与窒息程度呈正相关(P<0.05)。结论 血清HSP70、sICAM-1和MT因子与新生儿窒息程度密切相关,能有效反应新生儿窒息的严重程度,可作为检测新生儿窒息的重要指标,值得临床应用。【Objective】To observe the correlation between plasma levels of heat shock protein 70(HSP70), melatonin(MT),soluble cell adhesion molecules-1(sICAM-1) and neonatal asphyxia. 【Methods】From March 2019 to March 2021, the asphyxia neonates admitted to the Neonatology Department of our hospital were selected as the asphyxia group(74 cases), and the full-term healthy neonates were selected as the control group(67 cases). Umbilical arterial blood was collected from both groups of newborns within 24 hours of birth, and serum was separated to test the levels of HSP70, MT, and sICAM-1 in the two groups, the levels of HSP70, MT and sICAM-1 were compared between the two groups and in children with different degrees of neonatal asphyxia, and the correlation between HSP70, MT, sICAM-1 and Apgar score was analyzed by Pearson correlation. 【Results】The serum levels of HSP70 and sICAM-1 in the asphyxia group were significantly higher than those in the control group, and MT was significantly lower than that in the control group(P<0.05). Among the 74 infants with neonatal asphyxia, 32 cases had mild lesions(mild group),42 cases had severe lesions(severe group). Comparison of HSP70 and sICAM-1 between the two groups: mild groupsevere group, and the difference was statistically significant(P<0.05). The results of correlation analysis showed that HSP70 and sICAM-1 were negatively correlated with lesion degree, MT was positively correlated with lesion degree, and the differences were statistically significant(P<0.05). 【Conclusion】Serum HSP70,sICAM-1 and MT factors are closely related to the degree of neonatal asphyxia, which can effectively reflect the severity of neonatal asphyxia, and can be used as important indicators for detecting neonatal asphyxia, which is worthy of clinical application.
