摘 要:目的:观察术前颌间弹性牵引在颌骨骨折患者治疗中的临床效果。方法:选取2019年1月—2022年1月南京大学医学院附属盐城第一医院收治的新鲜颌骨骨折患者60例,随机分为对照组与观察组各30例,对照组行常规治疗;观察组除常规治疗外,术前给予颌间弹性牵引治疗,两组均完善检查,待面部肿胀消退后行钛板坚固内固定手术。评估术前牵引时间,两组手术时间、出血量,钛板、钛钉植入数量,术后颌间牵引时间、并发症,术后口腔功能及颞下颌关节功能恢复情况。结果:观察组平均术前颌间牵引(2.6±0.77)d,牵引2~3 d基本可恢复正常咬合关系。观察组手术时间及出血量显著低于对照组,术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。两组植入钛板、钛钉数量比较,差异无统计学意义(P>0.05)。术后颌间牵引所需时间观察组少于对照组,差异有统计学意义(P<0.05)。6个月后,两组张口功能及咬合关系均有效恢复,差异无统计学意义(P<0.05)。结论:术前颌间弹性牵引能短时间内有效恢复患者咬合关系,利于内固定手术进行,并减少手术时间、出血量及并发症发生率,缩短术后牵引时间,有效改善口腔功能且利于颞下颌关节功能恢复;值得临床应用。Objective To discuss the clinical application of preoperative intermaxillary elastic traction in the treatment of patients with jaw fracture.Method From January 2019 to January 2022,60 patients with fresh jaw fractures were admitted to Yancheng First Hospital Affiliated to Medical School of Nanjing University;They were randomly divided into two groups with 30 patients in each group,the control group received routine symptomatic treatment;In addition to the conventional symptomatic treatment,the observation group received intermaxillary elastic traction with traction nails before operation.both groups were examined thoroughly,and titanium plate fixation was performed after facial swelling subsided,Evaluating the preoperative traction time,the operation time,the amount of bleeding,the number of titanium plates and titanium nails implanted,the postoperative intermaxillary traction time,complications,the postoperative oral function and temporomandibular joint function recovery of the two groups.Results In the observation group,the average preoperative intermaxillary traction was 2.6±0.77 days,and the normal occlusion relationship could be basically restored after 2 to 3 days of traction.The operation time and bleeding volume in the observation group were significantly lower than those in the control group,and the postoperative complications were less(P<0.05).There was no significant difference in the number of implanted titanium plates and nails between the two groups(P>0.05).The time of intermaxillary traction in the observation group was lower than that in the control group(P<0.05).After 6 months,the mouth opening function and occlusion relationship of the two groups were effectively restored,with no significant difference(P>0.05);However,the score of temporomandibular joint function in the observation group was lower than that in the control group,with a statistically significant difference(P<0.05).Conclusion Preoperative intermaxillary elastic traction can effectively restore the occlusal relationship of patient
