摘　　要：目的 探讨采用尺骨鹰嘴入路双侧钢板螺钉内固定治疗严重肱骨远端粉碎性骨折的临床疗效。方法 2017年5月~2019年8月共收治52例肱骨远端粉碎性骨折患者,随机分为2组(A组和B组),每组26例,A组经尺骨鹰嘴入路治疗,B组经肱三头肌两侧入路治疗。术后摄肘关节X线片评估骨折愈合情况,通过Mayo肘关节功能评分评价临床疗效。结果 52例患者全部得到随访,平均3.6个月(3~6个月)达到骨性愈合,术后3个月时,肘关节活动范围均恢复正常。末次随访时,A组Mayo评分(89.2±2.8)分,B组Mayo评分(81.2±3.7)分,两组比较,差异有统计学意义(P<0.001);A组优良率92.3%,B组优良率88.5%,两组比较,差异有统计学意义(P<0.001)。结论 采用尺骨鹰嘴入路治疗严重的肱骨远端粉碎性骨折,固定可靠,临床效果显著。Objective To investigate the clinical efficacy of bilateral plate screw internal fixation of the ulnar olecranon approach in the treatment of severe distal humerus comminuted fractures.Methods From May 2017 to August 2019,a total of 52 patients with distal humerus comminuted fracture were randomly divided into two groups(group A and group B),with 26 cases in each group, group A was treated transulnar olecranon approach, and group B was treated bilateral approach through triceps brachii.Postoperative elbow X-ray was taken to evaluate fracture healing, and the clinical efficacy was evaluated by Mayo’s elbow function score.Results All 52 patients were followed up, and bony healing was achieved in an average of 3.6 months(3~6 months),and the range of motion of the elbow joint returned to normal three months after surgery.At the last follow-up, Mayo scores of group A and group B were 89.2±2.8 and 81.2±3.7,and the difference between the two groups was statistically significant(P<0.001).The excellent and good rate of group A was 92.3% and that of group B was 88.5%,and the difference between the two groups was statistically significant(P<0.001).Conclusion The ulna olecranon approach is reliable and effective in the treatment of severe comminuted distal humerus fracture.
