经阴道实时三维子宫输卵管超声造影剂逆流影响因素分析

摘　　要：目的 分析经阴道实时三维子宫输卵管超声造影过程中影响造影剂逆流的因素。方法 选取不孕症患者471例,行经阴道三维子宫输卵管超声造影检查,运用单因素分析和Logistic多因素分析等统计方法分析影响因素。结果 单因素分析中,影响TVS 3D-Hy CoSy造影剂逆流的因素有输卵管通畅程度(χ^(2)=12.869,P<0.05)、检查时限(χ^(2)=61.71,P<0.05)、内膜厚度(χ^(2)=6.54,P<0.05)和宫腔手术史(χ^(2)=4.397,P<0.05)。Logistic多因素分析中,影响TVS 3D-Hy CoSy造影剂逆流的因素为输卵管通畅程度(OR=0.028,95%CI 0.009-0.091,P=0.001)、内膜厚度(OR=1.859,95%CI 1.637-5.831,P=0.015)和宫腔手术史(OR=3.154,95%CI 2.364-9.541,P=0.026),检查时限有近似统计学意义(OR=2.742,95%CI 0.987-7.617,P=0.053)。结论 TVS 3D-Hy CoSy检查过程中,双侧输卵管通畅性、内膜厚度以及宫腔手术史都是引起造影剂逆流的因素。Objective To analyze the factors affecting contrast reversal during transvaginal real-time three-dimensional hysterosalpingography.Methods A total of 471 infertility patients were selected for transvaginal 3D hysterosalpingography, and the influencing factors were analyzed using statistical methods such as univariate analysis and logistic multi-factor analysis.Results In univariate analysis, the factors influencing TVS 3D-Hy CoSy contrast reversal were degree of tubal patency(χ^(2)=12.869,P<0.05),time frame of examination(χ^(2)=61.71,P<0.05),endometrial thickness(χ^(2)=6.54,P<0.05) and history of hysterectomy(χ^(2)=4.397,P<0.05).Logistic factors affecting TVS 3D-Hy CoSy contrast reversal in multifactorial analysis were degree of tubal patency(OR=0.028,95%CI 0.009-0.091,P<0.01),endometrial thickness(OR=1.859,95%CI 1.637-5.831,P=0.015) and history of hysterectomy(OR=3.154,95%CI 2.364-9.541,P=0.026),with a near statistically significant examination time frame(P=0.053).Conclusion Bilateral tubal patency, endometrial thickness, and history of hysterectomy are factors that cause contrast reflux during TVS 3D-Hy CoSy examination.
