疝环充填式无张力疝修补术与传统Bassini疝修补术治疗腹股沟疝的效果比较

摘　　要：目的:对比疝环充填式无张力疝修补术(PMHR)与传统Bassini疝修补术治疗腹股沟疝效果。方法:将88例腹股沟疝患者随机分为对照组(n=44)和观察组(n=44)。对照组予以传统Bassini疝修补术治疗,观察组予以PMHR治疗,术后随访1年。比较两组手术情况、基质金属蛋白酶水平变化、应激反应指标、疼痛评分、并发症发生情况及复发率。结果:观察组术中出血量、下床活动时间、住院时间均少于对照组;T-AOC、GH水平高于对照组;MMP-2、MMP-9、TIMP-1、TIMP-2、β-EP水平,以及术后1 d、3 d时疼痛评分、并发症发生率和复发率低于对照组(均P <0.05)。结论:PMHR治疗可减少腹股沟疝患者术中出血量,缩短可下床活动的时间,降低基质金属蛋白酶水平,减轻手术应激,且并发症少、复发率低。Objective:To compare the effects of plug mesh hernia repair(PMHR) and conventional Bassini hernia repair for inguinal hernia.Methods:Eighty-eight patients with inguinal hernia were randomly divided into a control group(n=44)and an observation group(n=44).The control group was treated with conventional Bassini hernia repair while the observation group was treated with PMHR,and the patients were followed up for 1 year after surgery.The operative conditions,changes in matrix metalloproteinase,stress response indicators,pain scores,complications and recurrence rates were compared between the two groups.Results:The volume of intraoperative blood loss,time to get out of bed and hospital stay were less and T-AOC and GH levels were higher in the observation group than the control group.MMP-2,MMP-9,TIMP-1,TIMP-2 and β-EP levels,as well as pain scores,complication rates and recurrence rates at postoperative day 1 and 3 were lower in the observation group than the control group(all P<0.05).Conclusion:PMHR treatment can reduce intraoperative bleeding,shorten the time to get out of bed,decrease the level of matrix metalloproteinases and surgical stress and has fewer complications and lower recurrence rate in patients with inguinal hernia.
