摘　　要：目的 分析载脂蛋白E(ApoE)、白细胞介素-8(IL-8)基因多态性与儿童肺炎支原体肺炎感染的关联性。方法 回顾性分析2018年8月-2021年8月重庆两江新区第一人民医院收治的156例肺炎支原体肺炎(MPP)患儿的临床资料(设为MPP组),并纳入经性别、年龄1∶1匹配的156名健康体检患儿作为对照组,比较两组ApoE基因rs7412、rs429358位点及IL-8基因rs4073、rs2227306位点基因型及基因频率分布,并分析ApoE及IL-8基因多态性与MPP患儿支气管黏膜形态学改变及预后转归的关系。结果 Hardy-Weinberg平衡检验提示各基因频率符合遗传平衡法则,具有较好群体代表性;MPP组ApoE基因rs429358位点CC型及等位基因C频率高于对照组(P<0.05),CC型患MPP的OR=3.694,携带等位基因C患MPP的OR=1.849;MPP组IL-8基因rs4073位点AA型及等位基因A频率高于对照组(P<0.05),AA型患MPP的OR=4.104,携带等位基因A患MPP的OR=1.457;ApoE基因rs429358位点及IL-8基因rs4073位点不同基因型支气管黏膜形态、预后转归时间差异均有统计学意义(P<0.05),其中ApoE rs429358位点CC基因型及IL-8 rs4073位点AA基因型黏液栓阻塞型表现最多(P<0.05),且退热时间、咳嗽消失时间、肺部啰音消失时间、住院时间更长(P<0.05)。结论 ApoE rs429358及IL-8 rs4073位点多态性与MPP易感性及患儿支气管黏膜形态、预后转归情况均有关,有望成为MPP诊疗的新靶点。OBJECTIVE To analyze the association of apolipoprotein E(ApoE) and interleukin-8(IL-8) gene polymorphisms with Mycoplasma pneumoniae pneumonia in children.METHODS The clinical data of 156 children with Mycoplasma pneumoniae pneumonia(MPP)(set as MPP group) between Aug.2018 and Aug.2021 in the First People′ s Hospital of Chongqing Liangjiang New Area were retrospectively analyzed,and 156 children with healthy physical examination matched by gender and age at 1∶1 were included as control group.The distribution characteristics of genotypes and gene frequencies at rs7412 locus and rs429358 locus of ApoE gene and at rs4073 locus and rs2227306 locus of IL-8 gene were compared between the two groups.The relationship between ApoE and IL-8 gene polymorphisms and bronchial mucosal morphological changes and prognosis and outcomes in children with MPP was analyzed.RESULTS The Hardy-Weinberg equilibrium test suggested that the frequencies of each gene were consistent with the law of genetic equilibrium and had a good population representation.The frequencies of CC genotype and allele C at rs429358 locus of ApoE gene in MPP group were higher than those in control group(P<0.05).The relative risk OR of MPP in patients with CC genotype was 3.694,and the OR of MPP in patients carrying allele C was 1.849.The frequencies of AA genotype and allele A at rs4073 locus of IL-8 gene were higher in MPP group than those in control group(P<0.05),and the OR of MPP in patients carrying AA genotype was 4.104 and the OR of MPP in patients carrying allele A was 1.457.There were significant differences in bronchial mucosal morphology and prognostic outcome time of different genotypes at rs429358 locus of ApoE gene and at rs4073 locus of IL-8 gene(P<0.05).The type of CC genotype at rs429358 locus of ApoE gene and AA genotype at rs4073 locus of IL-8 gene with mucus plug obstruction were the most(P<0.05),and the fever abatement time,cough disappearance time,pulmonary rales disappearance time and hospital stay were longer(P<0.05).CONCLUSION The po
