清肺化痰汤联合西医常规治疗呼吸机相关性肺炎老年患者临床研究

摘　　要：目的:观察清肺化痰汤联合西医治疗呼吸机相关性肺炎老年患者的临床疗效。方法:选取呼吸机相关性肺炎老年患者60例,随机分为对照组和治疗组各30例。对照组予常规抗感染、化痰、解痉平喘等治疗,治疗组在对照组的基础上给予清肺化痰汤内服。连续治疗2周后,比较2组临床肺部感染评分(CPIS)、急性生理学及慢性健康状况评分系统(APACHEⅡ)评分及总体临床疗效。结果:治疗组总有效率为96.67%,对照组为73.33%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组CPIS评分比较,差异无统计学意义(P>0.05);治疗后,2组CPIS评分均较治疗前降低(P<0.05),且治疗组CPIS评分低于对照组(P<0.05)。治疗前,2组APACHEⅡ评分比较,差异无统计学意义(P>0.05);治疗后,2组APACHEⅡ评分均较治疗前降低(P<0.05),且治疗组APACHEⅡ评分低于对照组(P<0.05)。结论:清肺化痰汤联合西医常规治疗老年呼吸机相关性肺炎可提高临床疗效,有效减轻肺部感染,改善患者健康状况。Objective:To observe the clinical effect of Qingfei Huatan Decoction combined with western medicine for senile patients with ventilator-associated pneumonia.Methods:A total of 60 cases of senile patients with ventilator-associated pneumonia were selected and randomly divided into the control group and the treatment group,with 30 cases in each group.The control group was given routine treatment for resisting infection,expelling phlegm and relieving muscular spasm and asthma,and the treatment group was additionally given oral administration of Qingfei Huatan Decoction based on the treatment of the control group.After 2-week treatment,the scores of Clinical Pulmonary Infection Score(CPIS)and Acute Physiology and Chronic Health EvaluationⅡ(APACHE-Ⅱ)and the total clinical effect were compared between the two groups.Results:The total effective rate was 96.67% in the treatment group and 73.33% in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of CPIS scores between the two groups(P>0.05).After treatment,the CPIS scores in the two groups were decreased when compared with those before treatment(P<0.05),and the CPIS score in the treatment group was lower than that in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of APACHEⅡ scores between the two groups(P>0.05).After treatment,the APACHEⅡ scores in the two groups were decreased when compared with those before treatment(P<0.05),and the APACHEⅡ score in the treatment group was lower than that in the control group(P<0.05).Conclusion:Qingfei Huatan Decoction combined with routine treatment in western medicine for senile patients with ventilator-associated pneumonia can improve clinical effect,effectively relieve pulmonary infection and improve health condition.
