郭氏苦柏方坐浴治疗肛瘘术后患者疗效观察及对创面的影响

摘　　要：目的:观察郭氏苦柏方坐浴治疗肛瘘术后患者的临床疗效及其对创面的影响。方法:选择低位单纯性肛瘘患者120例,随机分为研究组与对照组各60例。对照组给予高锰酸钾溶液坐浴治疗,研究组采用郭氏苦柏方煎汤坐浴治疗。2组疗程均为3周。比较2组疗效、创面疼痛视觉模拟评分法(VAS)评分、创面临床指标评分、肉芽评分、创面愈合率、创面毛细血管数量、创面血管内皮细胞生长因子(VEGF)表达水平。结果:研究组总有效率为95.0%,对照组为75.0%,2组比较,差异有统计学意义(P<0.05)。术后3、7、21 d,研究组创面疼痛、创面临床指标及创面肉芽评分均低于对照组,差异有统计学意义(P<0.05)。术后7、14、21 d,研究组创面愈合率均高于对照组,差异有统计学意义(P<0.05)。术后7、21 d,研究组毛细血管数量均多于对照组,差异有统计学意义(P<0.05)。术后7、14 d,研究组创面VEGF阳性强度高于对照组,差异有统计学意义(P<0.05)。结论:在肛瘘术后创面恢复期给予郭氏苦柏方联合坐浴治疗,可提高临床疗效,减轻患者疼痛,有效促进创面愈合。Objective:To observe the clinical effect of sit-baths with Guo’s Kubai Formula on patients after surgery for anal fistula and its effect on the wound.Methods:A total of 120 patients with low simple anal fistula were randomly divided into two groups,with 60 patients in each group.The control group was given hip bath with potassium permanganate solution,and the study group was given hip bath with decoction of Guo’s Kubai Formula.The treatment course of the two groups was 3 weeks.The clinical effects,scores of Visual Analogue Scale(VAS),wound clinical indexes scores,granulation scores,wound healing rates,the wound capillaries number and the levels of wound vascular endothelial growth factors(VEGF)were compared between the two groups.Results:The total effective rate was 95.0% in the study group,and 75.0% in the control group,the difference being significant(P<0.05).At 3 days,7 days,and 21 days after the surgery,the wound pain,wound clinical indexes and wound granulation scores in the study group were lower than those in the control group,the difference being significant(P<0.05).At7 days,14 days,and 21 days after the surgery,the wound healing rate in the study group was higher than that in the control group,the difference being significant(P<0.05).At 7 days and 21 days after the surgery,the number of capillaries in the study group was larger than that in the control group,the difference being significant(P<0.05).At 7 and 14 days after the surgery,the positive intensity of VEGF in the wound in the study group was higher than that in the control group,the difference being significant(P<0.05).Conclusion:The application of hip bath with Guo’s Kubai Formula can improve the curative effect on recovering the wound of patients after surgery for anal fistula,reduce their pain,and promote the wound healing.
