益肾接骨汤对老年四肢创伤性骨折术后关节功能、骨密度及骨折愈合速度的影响

摘　　要：目的:观察益肾接骨汤联合碳酸钙D3片对老年四肢创伤性骨折术后患者关节功能、骨密度及骨折愈合速度的影响。方法:选取60例老年四肢创伤性骨折患者,采用随机数字表法分为观察组和对照组各30例。2组均进行钢板螺钉内固定术治疗,术后给予头孢氨苄片治疗3 d,对照组在此基础上给予口服碳酸钙D3片治疗,观察组加予益肾接骨汤治疗。2组均治疗1个月,随访6个月。比较2组临床疗效、关节功能、骨密度及骨折愈合速度。结果:治疗1个月,观察组临床疗效总有效率93.33%,高于对照组73.33%(P<0.05)。随访6个月,2组Mayo肘关节评分、美国纽约特种外科医院(HSS)膝关节评分均较术前升高(P<0.05),观察组Mayo肘关节评分、HSS膝关节评分均高于对照组(P<0.05)。观察组关节功能优良率86.67%,高于对照组63.33%(P<0.05)。2组骨密度均较术前升高(P<0.05),观察组骨密度高于对照组(P<0.05)。观察组骨折愈合时间短于对照组(P<0.05)。结论:老年四肢创伤性骨折术后给予益肾接骨汤联合碳酸钙D3片治疗,可促进患者关节功能恢复,提高其骨密度和骨折愈合速度。Objective:To observe the effect of Yishen Jiegu Decoction combined with Calcium Carbonate-Vitamin D3Tablets on postoperative joint function,bone density,and fracture healing speed in senile patients with traumatic extremities fractures.Methods:A total of 60 cases of senile patients with traumatic extremities fractures were selected and divided into the observation group and the control group according to the random number table method,with 30 cases in each group.Both groups were given internal fixation with plate and screw.After the surgery,both groups were treated with Cefalexin Tablets for 3 days,the control group was additionally treated with oral administration of Calcium Carbonate-Vitamin D3Tablets,and the observation group was additionally treated with Yishen Jiegu Decoction.The treatment in the two groups lasted for 1 month,and the follow-up lasted for 6 months.Clinical effects,joint function,bone density,and fracture healing speed were compared between the two groups.Results:After 1 month of treatment,the total effective rate of clinical effect was 93.33% in the observation group,higher than that of 73.33% in the control group(P<0.05).After the 6-month follow-up,Mayo Elbow Performance scores and the knee joint scores in Hospital for Special Surgery(HSS)in the two groups were increased when compared with those before operation(P<0.05),and the above scores in the observation group were higher than those in the control group(P<0.05).The Excellent rate of joint function was 86.67% in the observation group,higher than that of 63.33% in the control group(P<0.05).Bone density in the two groups were increased when compared with those before operation(P<0.05),and the bone density in the observation group was higher than that in the control group(P<0.05).The fracture healing time in the observation group was shorter than that in the control group(P<0.05).Conclusion:Yishen Jiegu Decoction combined with Calcium Carbonate-Vitamin D3Tablets for senile patients after surgery for traumatic extremities fractures can promo
