尪痹胶囊联合牵引疗法治疗腰椎间盘突出症伴坐骨神经痛临床研究

摘　　要：目的:观察尪痹胶囊联合牵引疗法治疗腰椎间盘突出症伴坐骨神经痛的效果。方法:将60例肝肾不足、风湿阻络型腰椎间盘突出症伴坐骨神经痛患者随机分为试验组和对照组各30例,2组患者均接受牵引治疗,试验组加服尪痹胶囊。2组均治疗28 d。治疗前后评估2组患者臀部、大腿后侧、小腿后外侧等坐骨神经区域内的疼痛视觉模拟评分法(VAS)评分、日本矫形外科学会(JOA)下腰痛评分,统计2组的临床疗效和不良反应发生情况,评估其安全性。结果:治疗后,临床疗效总有效率试验组93.33%,对照组66.67%,2组比较,差异有统计学意义(P<0.05)。2组JOA下腰痛评分均较治疗前升高(P<0.05);试验组JOA下腰痛评分高于对照组(P<0.05)。2组坐骨神经分布区的疼痛VAS评分均较治疗前下降(P<0.05);试验组疼痛VAS评分低于对照组(P<0.05)。试验组出现1例肝功能损害,服用保肝药后好转,未见其余与治疗相关的不良反应。结论:应用尪痹胶囊联合牵引疗法治疗腰椎间盘突出症伴坐骨神经痛患者可切实减轻腰痛及坐骨神经痛等相关症状,安全性较好,能促进腰椎间隙恢复正常,加快康复。Objective:To observe the effect of Wangbi Capsules combined with distraction therapy on lumbar disc herniation complicated with sciatica.Methods:A total of 60 cases with lumbar disc herniation complicated with sciatica of the type of liver-kidney insufficiency and wind-damp obstructing the collaterals were randomly divided into the trial group and the control group,with 30 cases in each group.The two groups were treated with distraction therapy and the trial group was additionally treated with Wangbi Capsules.Both groups were treated for 28 days.The scores of Visual Analogue Scale(VAS)and Japanese Orthopaedic Association(JOA)of low back pain were evaluated before and after treatment in the two groups.The clinical effects and the occurrence of adverse reactions in the two groups were analyzed,and their safety was evaluated.Results:After treatment,the total effective rate of clinical effect was 93.33% in the trial group and 66.67% in the control group,the difference being significant between the two groups(P<0.05).The scores of JOA low back pain in both groups were increased when compared with those before treatment(P<0.05);the score of JOA low back pain in the trial group was higher than that in the control group(P<0.05).The VAS scores of pain in sciatic nerve distribution in both groups were decreased when compared with those before treatment(P<0.05);the VAS score of pain in the trial group was lower than that in the control group(P<0.05).In the trial group,one patient had liver function impairment,which was improved after taking hepatoprotective medicine,and no other treatment-related adverse reactions were observed.Conclusion:The application of Wangbi Capsules combined with distraction therapy can effectively relieve the low back pain,sciatica and other related symptoms in patients with lumbar disc herniation complicated with sciatica,which is safe and can promote the restoration of lumbar interstitial space and speed up recovery.
