经阴道彩色多普勒超声心动图诊断宫颈部胎盘植入的效果

摘　　要：目的 探讨经阴道彩色多普勒超声心动图诊断宫颈部胎盘植入的效果。方法 选取2016年7月至2021年11月于医院就诊的49例疑似宫颈部胎盘植入患者为研究对象,均给予经阴道彩色多普勒超声心动图检查,记录影像学特征,并给予病理检查,判断诊断价值。结果 49例疑似宫颈部胎盘植入患者中,病理结果提示宫颈部胎盘植入患者21例(胎盘植入组),非宫颈部胎盘植入患者28例(非胎盘植入组)。宫颈部胎盘植入患者中粘连型10例、植入型7例、穿透型4例;胎盘植入组年龄、孕次、产次、流产史等与非胎盘植入组比较,差异有统计学意义(P<0.05),两组孕周与合并前置胎盘情况比较,差异无统计学意义(P>0.05)。49例患者中,经阴道彩色多普勒超声心动图诊断宫颈部胎盘植入21例,特异度为92.9%(26/28),灵敏度为90.5%(19/21);经阴道彩色多普勒超声心动图诊断粘连型11例,植入型6例,穿透型3例,分型诊断准确度为81.6%(40/49)。结论 经阴道彩色多普勒超声心动图诊断宫颈部胎盘植入敏感度、特异度与准确度较高,可对患者进行植入分型判定。Objective The effects of transvaginal color Doppler echocardiography in the diagnosis of cervical placenta accreta was discussed.Methods From July 2016 to November 2021,49 cases of patients with suspected cervical placenta accreta who were treated in our hospital were selected as the research subjects.All patients were examined by transvaginal color Doppler echocardiography,and the imaging characteristics were recorded.All patients were given pathological examination and judged the diagnostic value.Results In the 49 patients with suspected cervical placenta accreta,there were 21 patients with cervical placenta accreta(placenta accreta group) and 28 patients with non-cervical placenta accreta(non-placenta accreta group)were found by pathological results.In the patients with cervical placenta accreta,there were 10 cases,7 cases and 4 cases of adhesion type,implantation type and penetration type,respectively.There were significant differences in age,pregnancy,parity,and history of miscarriage compared between the placenta accreta group and the non-placenta accreta group(P0.05).In the 49 patients,there were 21 cases were diagnosed with cervical placenta accreta by transvaginal color Doppler echocardiography,with a specificity of 92.9%(26/28) and the sensitivity was 90.5%(19/21),the coincidence rate was 91.8%(45/48).Transvaginal color Doppler echocardiography was used to diagnose adhesion type in 11 cases,implantation type in 6 cases and penetration type in 3 cases,with the diagnostic accuracy of 81.6%(40/49).Conclusion Transvaginal color Doppler echocardiography has high sensitivity,specificity and accuracy in the diagnosis of cervical placenta accreta,and it can also be used to determine the type of implantation in patients.
