摘　　要：目的:探讨超声引导下微创单通道经皮肾镜钬激光碎石术治疗肾结石的临床效果。方法:选择2018年1月-2022年9月邳州市人民医院收治的68例肾结石患者参与此次研究,以随机数字表法分为对照组(34例,常规开放性手术治疗)和治疗组(34例,超声引导下微创单通道经皮肾镜钬激光碎石术治疗)。比较两组手术指标、肾功能指标、并发症发生情况及术后3 d结石清除率。结果:治疗组较对照组手术时间更短,术中出血量更少,疼痛评分更低,差异有统计学意义(P<0.001)。治疗前,两组血肌酐、尿素氮、β_(2)-微球蛋白水平比较,差异无统计学意义(P>0.05);治疗后,治疗组血肌酐、尿素氮、β_(2)-微球蛋白水平低于对照组,差异有统计学意义(P<0.001)。治疗组并发症发生率2.88%,较对照组的23.53%更低,术后3 d结石清除率97.06%,较对照组76.47%更高,差异有统计学意义(P<0.05)。结论:超声引导下微创单通道经皮肾镜钬激光碎石术治疗肾结石的创伤小,可有效改善肾功能,降低术后并发症发生率。Objective:To investigate the clinical effect of ultrasound guided minimally invasive single channel percutaneous nephrolithotomy with holmium laser in the treatment of renal calculi.Methods:A total of 68 patients with renal calculi admitted to our hospital(January 2018 to September 2022) were selected to participate in this study.They were randomly divided into the control group(34 patients,treated with conventional open surgery) and the treatment group(34 patients,treated with ultrasound guided minimally invasive single channel percutaneous nephrolithotomy with holmium laser).The surgical indicators,renal function indicators,complications and stone clearance rate 3 days after surgery were compared between the two groups.Results:the treatment group had a shorter operation time,less intraoperative blood loss and lower pain score compared with the control groupt(P>0.05).After treatment,the levels of blood creatinine,urea nitrogen and β_(2)-microglobulin in the treatment group were lower than those in the control group(P<0.001).The complication rate in the treatment group was 2.88%,lower than 23.53% in the control group,and the stone clearance rate at 3 d after surgery was 97.06% and higher than 76.47% in the control group(P<0.05).Conclusion:Ultrasound guided minimally invasive single channel percutaneous nephrolithotomy with holmium laser is less invasive in the treatment of renal calculi,which can effectively improve renal function and reduce the incidence of postoperative complications.
