柴胡类方的临床应用经验

摘　　要：随着现代社会的高速发展,人们由于饮食不节、起居失常、劳累过度、竞争压力增加,普遍出现暗耗精血的情况,又因当代人多情绪急躁易怒,肝气郁结常见,导致气机升降失常,出现外可见寒热往来,肢节烦痛,内可见默默不欲饮食,胃脘腹部胀满疼痛,便秘或腹泻,或小便不利,更多则见心烦、胸胁苦满喜叹息、失眠多梦、烦躁不安、眩晕气短等,整体病机为血弱气尽,腠理开,邪气因入,与正气相搏。以上诸症,多为柴胡类方的适应证。柴胡类方包括大小柴胡汤、柴桂姜汤、柴胡桂枝汤、柴胡加龙骨牡蛎汤,共同病机条畅气血津液,和解三焦。外可祛六淫之邪,内可调和诸多慢性杂病,起到气血调和,阴平阳秘的功效。With high-speed development of modern society,as a result of improper diet,irregular daily life,overwork and increasing pressure of competition,the surreptitious consumption of blood and essence is prevalent.In addition,most of the modern people have impatience and irritability,as well as common stagnation of liver qi,leading to ascent-descent disorders of qi movement.The external manifestations include alternate attacks of chill and fever,and restless arthralgia of limbs,while the internal manifestations include the retching with poor appetite,the distention,fullness and pain of gastric cavity and abdomen,constipation or diarrhea,or dysuria,and the more commonly observed manifestations include upset,fullness and discomfort in chest and hypochondrium with sighing,insomnia and dreaminess,dysphoria,dizziness,and shortness of breath.The overall pathogenesis is the deficiency of qi-blood weakness,the opening of striae,the invasion of pathogenic factors and its interaction with vital qi.Most of the above symptoms are indications for Chaihu prescriptions.Chaihu prescriptions include Da-Xiao Chaihu Decoction,Chaihu Guizhi Ganjiang Decoction,Chaihu Guizhi Decoction,Chaihu Plus Longgu Muli Decoction.The common pathogenesis is adjusting the balance between essence,qi,blood and body fluids,and reconciling Sanjiao.The treatment can dispel the six climatic conditions in excess as pathogenic factors,regulate many chronic miscellaneous diseases,and play a role in harmonizing the coordination between qi and blood,and relative equilibrium of yin-yang.
