身痛逐瘀汤联合椎板间入路脊柱内镜下椎间盘摘除术治疗血瘀气滞型L_(5)S_(1)腰椎间盘突出症的效果

摘　　要：目的:探讨身痛逐瘀汤结合椎板间入路脊柱内镜下椎间盘摘除术(PEID)治疗血瘀气滞型L_(5)S_(1)腰椎间盘突出症(LDH)的临床效果。方法:选取2020年9月-2021年9月北京市昌平区中西医结合医院收治的80例血瘀气滞型L_(5)S_(1)LDH患者作为研究对象,以随机数字表法分为对照组与观察组,各40例。对照组采用PEID手术治疗,观察组在对照组基础上配合口服身痛逐瘀汤治疗,比较两组疼痛评分、功能障碍评分、手术优良率。结果:治疗前,两组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);术后1、3个月,两组VAS评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组日本骨科协会评估治疗分数(JOA)评分比较,差异无统计学意义(P>0.05);术后1、3个月,两组JOA评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。两组手术优良率比较,差异无统计学意义(P>0.05)。结论:身痛逐瘀汤联合PEID治疗血瘀气滞型L_(5)S_(1)LDH患者有效显著,可有效缓解患者术后疼痛,改善腰椎功能。Objective:To explore the clinical effect of Shentong Zhuyu Decoction combined with percutaneous endoscopic interlaminar discectomy(PEID)for L5S1 lumbar disc herniation(LDH)with blood stasis and qi stagnation.Methods:From September 2020 to September 2021,a total of 80 L5S1 LDH patients with blood stasis and qi stagnation who were treated in Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 40 cases in each group.The control group was treated with PEID surgery,and the observation group was treated with oral administration of Shentong Zhuyu Decoction on the basis of the control group.The pain score,dysfunction score,and excellent and good surgery rate between the two groups were compared.Results:Before treatment,there was no significant difference in visual analogue scale(VAS)score between the two groups(P>0.05).At 1 and 3 months after surgery,the VAS scores in two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the Japanese Orthopaedic Association(JOA)score between the two groups(P>0.05).At 1 and 3 months after surgery,the JOA scores in two groups were higher than those before treatment,and the observation group was higher than the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the excellent and good surgery rate between the two groups(P>0.05).Conclusion:Shentong Zhuyu Decoction combined with PEID in the treatment of L5S1 LDH patients with blood stasis and qi stagnation has significant effects,it can effectively relieve postoperative pain and improve lumbar spine function.
