摘　　要：目的:探讨产前阶段左无名静脉在超声筛查胎儿上腔静脉先天变异中的价值。方法:选取2019年1月-2021年12月深圳市宝安区石岩人民医院收治的48例胎儿存在上腔静脉先天性变异的孕妇资料进行回顾性分析,48例孕妇接受系统全面的产前超声筛查,并应用彩色多普勒能量技术,对胎儿的上腔静脉先天性变异超声检查特征予以分析。结果:胎儿上腔静脉先天变异类型主要分为4类,永存左上腔36例,占75.00%;永存左上腔伴右上腔缺如1例,占2.08%;主动脉弓左后无名静脉8例,占16.67%;左无名静脉胸腺内走行3例,占6.25%。结论:在上腔静脉先天变异胎儿中,其三血管气管切面在左无名静脉超声产前筛查中图像表现的特异性较高,诊断准确率较为理想,可将其作为筛查胎儿先天上腔变异的关键评判指标和方法。Objective:To investigate the value of prenatal left innominate vein in ultrasound screening of congenital variation of fetal superior vena cava.Methods:From January 2019 to December 2021,the data of 48 pregnant women whose fetuses had superior vena cava congenital variation and were admitted to Shiyan People’s Hospital of Baoan District of Shenzhen were retrospectively analyzed,48 pregnant women first received systemic and comprehensive prenatal ultrasound screening.Color Doppler energy technology was used to analyze the ultrasonographic characteristics of different types of congenital variation of the superior vena cava in the fetus.Results:The congenital variation of superior vena cava in fetus was mainly divided into four types:persistent left superior cavity in 36 cases,accounting for 75.00%;persistent left superior cavity with absence of right superior cavity in 1 case,accounting for 2.8%;left posterior innominate vein of aortic arch in 8 cases,accounting for 16.67%;left innominate vein in thymus in 3 cases,accounting for 6.25%.Conclusion:In the fetus with congenital variation of superior vena cava,the three-vessel trachea section of the fetus has higher image performance specificity and better diagnostic accuracy in the ultrasound prenatal screening of the left innominate vein.It can be used as a key evaluation index and method for screening the fetus with congenital variation of superior vena cava.
