摘　　要：胃为传送水谷之通道,以通为用,以降为顺,喜润而恶燥,凡能疏调气机,恢复胃腑和顺通降职能之法皆为通法,通法蕴含虚实平允、寒热平调、气血平和、阴阳平秘之意。胃癌病机为本虚标实,本虚为脾胃虚弱,标实为癌毒留滞,本质是“不通”。初期气血瘀滞、痰凝互结,以标实为主;后期则正气虚馁,以本虚为主。应在健脾助运,补益后天的基础上,佐以清热解毒、理气活血、化痰散结、燥湿坚阴等法。健脾补虚用四君子汤、补中益气汤等;湿热蕴结脾胃用藤梨根、无花果、薏苡仁、黄连温胆汤、苍术、黄连、苦参等;平调寒热用半夏泻心汤;气虚血瘀者加补气活血化瘀药物;病程后期,加益气养阴药物。The stomach is in the position of the six-fu organs,and is the channel to transmit water and grain.It is unobstructed in function,and smooth in descending.It likes to be moistened and hates dryness.Any method that can dredge and regulate qi movement,restore the the stomach function and smooth the passage can belong to dredging method,which contains regulating deficiency and excess,balancing cold and heat,harmonizing qi and blood,balancing yin and yang.The pathogenesis of stomach cancer is spleen-stomach deficiency as the deficient root cause,and retention of cancer toxin as the excessive manifestation,and the essence is "obstruction".In the initial stage,qi and blood stasis,intermingled phlegm and coagulation are excessive manifestations.In the later stage,the deficiency of healthy qi is the deficient root cause.So it should be treated by invigorating the spleen to help transport,tonifying the acquired qi,and then by clearing heat and detoxicating,regulating qi and activating blood,resolving phlegm and dispersing stasis,drying dampness and nourishing yin.For invigorating the spleen and tonifying the deficiency,Four Gentlemen Decoction,Center-Supplementing and Qi-Boosting Decoction,etc.can be used.For dampness-heat accumulation in the spleen and stomach,Tengligen(Actinidia Arguta),Wuhuaguo(Ficus Carica Linn),Yiyiren(Semen Coicis),Cangzhu(Rhizoma Atractylodis),Huanglian(Rhizoma Coptidis),Kushen(Radix Sophorae Flavescentis),Coptis Gallbladder-Warming Decoction,etc.can be used.For regulating cold and heat,drugs for invigorating qi and activating blood circulation and removing blood stasiscan be used.In the later stage of the disease,drugs for replenishing qi and nourishing yin should be added.
