伏气消癜汤治疗儿童过敏性紫癜血热妄行证疗效观察

摘　　要：目的:观察伏气消癜汤治疗儿童过敏性紫癜血热妄行证的临床疗效及其对免疫学指标的影响。方法:选取2020年6月至2022年1月南阳市第二人民医院收治的儿童过敏性紫癜血热妄行证患儿70例,随机分为对照组和试验组,每组各35例,对照组给予维生素C片、氯雷他定片、碳酸钙D3颗粒等治疗,试验组在对照组治疗的基础上加入伏气消癜汤,日1剂,水煎服,早晚温服。两组患儿均连续治疗2周。观察两组患儿临床疗效及皮肤消退、腹痛消失、尿常规恢复正常时间;比较两组患儿治疗前后T细胞亚群水平、中医证候积分、紫癜分布积分、紫癜颜色积分和紫癜形态积分变化情况,比较半年后复发情况等。结果:对照组有效率为80.00%,试验组有效率为97.14%,两组有效率比较,差异有统计学意义(P<0.05);试验组皮疹消失时间、腹痛消失时间、尿常规恢复正常时间均短于对照组,差异有统计学意义(P<0.05);治疗后两组患儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,CD8^(+)低于治疗前,且组间比较,差异有统计学意义(P<0.05);治疗后两组患儿中医证候积分、紫癜分布积分、紫癜颜色积分、紫癜形态积分均低于治疗前,且组间比较,差异有统计学意义(P<0.05);对照组复发率为17.14%,试验组复发率为5.71%,两组复发率比较,差异有统计学意义(P<0.05)。结论:伏气消癜汤治疗儿童过敏性紫癜血热妄行证可缓解患儿症状,调节患儿免疫功能,降低复发率。Objective:To observe the clinical efficacy of Decoction for Removing Latent Purpurain treating children suffering allergic purpura with syndrome of reckless flow of blood due to heat and its influence on immunological indexes.Methods:A total of 70 children suffering allergic purpura with syndrome of reckless flow of blood due to heat admitted to our hospital from June 2020 to January 2022 were randomly divided into the control group and the experimental group,with 35 cases in each group.The control group was treated with vitamin C tablets,Loratadine Tablets,Calcium Carbonate D3 Granules,etc.The experimental group was added with Decoction for Removing Latent Purpura on the basis of the treatment of the control group,one dose each day,decocted in water,and warm taken in the morning and evening.Both groups were treated for 2 weeks.Then the clinical efficacy,the time for skin to subside,abdominal pain to disappear,and urine routine to return to normal,T cell subgroup level,TCM syndrome score,purpura distribution score,purpura color score and purpura shape score,recurrence after half a year of the two groups were observed.Results:The effective rate of the control group was 80.00%,and that of the experimental group was 97.14%,and the difference between the two groups was statistically significant(P<0.05).The disappearance time of skin rash,abdominal pain and normal urine routine in the experimental group was all shorter than that in the control group,and the difference between the two groups was statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+) in the two groups were higher than those before treatment,while the level of CD8^(+) was lower than that before treatment,and all the differences between the two groups were statistically significant(P<0.05);The TCM syndrome score,purpura distribution score,purpura color score and purpura shape score of the two groups were lower than those before treatment,and all the differences between the two groups were statistically significant(
