针药结合辨治肠易激综合征临床观察

摘　　要：目的:观察针药结合辨治肠易激综合征腹泻型(irritable bowel syndrome diarrhea type,IBS-D)脾肾阳虚证的临床疗效。方法:选取2021年12月至2022年6月安徽中医药大学第二附属医院消化科诊治的IBS-D脾肾阳虚证患者60例,随机分为治疗组和对照组,每组30例。对照组给予暖中固元方加减治疗,治疗组给予针药结合治疗。观察两组患者治疗前后中医证候总评分、各项评分、焦虑自评量表(self rating anxiety scale,SAS)评分变化情况及临床疗效。结果:两组治疗后中医证候总评分、各项评分、SAS评分均低于治疗前,且治疗组治疗后中医证候总评分、各项评分、SAS评分低于对照组,差异具有统计学意义(P<0.05);治疗组有效率为90.00%,对照组有效率为73.33%,治疗组有效率高于对照组,差异有统计学意义(P<0.05);两组患者治疗期间均未发现明显不良反应。结论:针药结合辨治IBS-D脾肾阳虚证能明显改善患者腹痛、腹泻、纳呆、腰膝酸软、畏寒、神疲乏力、少气懒言等症状。Objective:To observe the clinical efficacy of acupuncture combined with medicine in the treatment of spleen-kidney yang deficiency syndrome of irritable bowel syndrome diarrhea type(IBS-D).Methods:A total of 60 patients with spleen kidney yang deficiency syndrome of IBS-D diagnosed and treated in the Department of Gastroenterology of the Second Affiliated Hospital of Anhui University of Traditional Chinese Medicine from December 2021 to June 2022 were randomly divided into the treatment group and the control group,with 30 cases in each group.The control group was treated with Modified Decoction for Warming the Middle Energizer and Strengthening Original Qi,while the treatment group was treated with acupuncture combined with medicine.The changes of the total score of TCM syndrome,various scores,self rating anxiety scale(SAS) scores and clinical efficacy of the two groups of patients before and after treatment were observed.Results:After treatment,the total score of TCM syndrome,each score and SAS score of the two groups were lower than those before treatment,and the total score of TCM syndrome,each score and SAS score of the treatment group were lower than those of the control group,and all the differences were statistically significant(P<0.05).The effective rate of the treatment group was 90.00%,and that of the control group was 73.33%.The effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).No obvious adverse reaction was found in both groups during treatment.Conclusion:Combination of acupuncture and medicine in differentiation and treatment of spleen-kidney yang deficiency syndrome of IBS-D can significantly improve the symptoms of patients such as abdominal pain,diarrhea,dizziness,waist and knee soreness,fear of cold,mental fatigue and fatigue,lack of qi and reluctance to speak,etc.
