湿润烧伤膏在低位单纯性肛瘘术后创面中的应用效果分析

摘　　要：目的研究探讨湿润烧伤膏在低位单纯性肛瘘术后创面中的应用效果。方法选取2019年6月至2020年10月武汉市第一医院收治的60例拟行肛门瘘管切除术治疗的低位单纯性肛瘘患者作为研究对象,并按照不同治疗方法将其分为试验组(30例)与对照组(30例),试验组患者肛门瘘管切除术后创面采用湿润烧伤膏换药治疗,对照组患者肛门瘘管切除术后创面采用凡士林油纱换药治疗,对比观察两组患者创面肉芽组织生长、水肿、疼痛与炎症因子水平变化情况。结果术后第7天,试验组患者创面肉芽组织生长、水肿及视觉模拟评分法(VAS)评分均明显低于对照组(t=3.272、2.762、3.795,P=0.002、P=0.008、P<0.001);血清白细胞介素(IL)-4水平明显高于对照组(t=4.608,P<0.001),IL-17及肿瘤坏死因子-α(TNF-α)水平均明显低于对照组(t=3.921、3.047,P<0.001、P=0.004)。结论与凡士林油纱换药治疗相比,湿润烧伤膏可明显减轻低位单纯性肛瘘患者术后创面疼痛及水肿程度,抑制炎症反应,促进创面肉芽组织生长,加快创面愈合。Objective To investigate the curative effect of MEBO in the postoperative wound of simple low anal fistula.Methods 60 patients with simple low anal fistula,admitted to Wuhan No.1 Hospital from June 2019 to October 2020 for anal fistula resection,were enrolled as research subjects,and then divided into experiment group(n=30),receiving dressing change of MEBO after anal fistula resection,and control group(n=30),receiving dressing change of Vaseline gauze after anal fistula resection.Granulation tissue growth,edema,pain and the level changes of inflammatory factors were observed and compared between the two groups.Results On day 7 after operation,the granulation tissue growth,edema and visual analogue scale(VAS)scores were all significantly lower in experiment group compared with control group(t=3.272,2.762 and 3.795,P=0.002,P=0.008,P<0.001),the level of serum interleukin(IL)-4 was significantly higher(t=4.608,P<0.001)while the levels of IL-17 and tumor necrosis factor-α(TNF-α)were both significantly lower(t=3.921 and 3.047,P<0.001,P=0.004)in experiment group compared with control group.Conclusion Compared with the dressing change of Vaseline gauze,MEBO is better in alleviating the degree of postoperative pain and edema in patients with simple low anal fistula,inhibiting inflammatory reaction,promoting the growth of wound granulation tissues and accelerating wound healing.
