摘 要:目的 探讨超声评估健康成人面动脉分型的可行性及应用价值,为颌面部整形手术提供影像学参考。方法 应用超声对100例健康志愿者的双侧面动脉进行扫查,观察面动脉全程走行并根据终末分支位置进行分型,测量并记录下颌骨下缘处面动脉内径,分析两名检查医师在评估分型和测量内径上的一致性。结果 根据面动脉终末分支位置分为四型:Ⅰ型(上唇型)27条,Ⅱ型(鼻型)124条,Ⅲ型(经典型)39条,Ⅳ型(双重型)10条,分型在性别、左右侧及年龄组中分布无统计学意义(P>0.05);下颌骨下缘处面动脉内径男性均值(1.73±0.35)mm,女性均值(1.42±0.22)mm,两者之间差异有统计学意义(P<0.05);两名检查医师在评估分型上具有较好的观察者间一致性(Kappa值>0.8),在测量内径上具有较好的观察者间和观察者内一致性(组内相关系数ICC值均>0.75)。结论 超声能够清晰显示面动脉全程走行并准确分型,超声分型可为颌面部相关手术提供临床参考。Objective To explore the feasibility and application value of ultrasound in evaluating the classification of facial artery in healthy adults, and provide imaging evaluation for maxillofacial plastic surgery. Methods Bilateral facial arteries of 100 healthy volunteers were scanned by ultrasound. The whole course of facial artery was observed and classified according to its terminal branch, the inner diameter of facial artery at the mandibular border was measured and recorded, and the consistency of the two sonographers in type classifying and inner diameter measuring was analyzed. Results There were 27 cases of typeⅠ(upper lip),124 cases of type Ⅱ(nasal),39 cases of type Ⅲ(classic)and 10 cases of type Ⅳ(double),which categorized according to the terminal branch of the facial artery. There was no statistical difference in the type distribution between gender, sides and age group(P>0.05). The inner diameters of facial arteries were(1.73±0.35) mm in males and(1.42±0.22) mm in females, which showed significant statistical difference(P<0.05). There was a good inter-observer consistency between two sonographers in type evaluation(Kappa>0.8),and there were good inter-observer and intra-observer consistency in inner diameter measurement(all intraclass correlation coefficient ICC>0.75). Conclusions Ultrasound can show the whole course of facial artery clearly and can be used to type classify accurately. Ultrasonic classification can provide clinical reference for maxillofacial surgery.
