经阴道彩色多普勒超声成像对卵巢早衰的诊断及血流动力学特征分析

摘　　要：目的 探讨经阴道彩色多普勒超声成像对卵巢早衰患者的诊断价值,并分析其血流动力学特征。方法 接受治疗的卵巢早衰患者102例为POF组,以1∶1配对选择同时期进行健康体检的健康女性102例为对照组。两组女性均接受经阴道彩色多普勒超声检查,比较超声声像图特征和血流动力学特征,采用ROC曲线分析超声声像图特征对卵巢早衰诊断预测的准确度。结果 POF组患者闭经时间6~19个月,平均(11.08±2.23)个月。POF组患者卵巢体积(OV)、卵泡数(AFC)、卵巢血管化指数(VI)、血流指数(FI)显著低于对照组(P<0.05)。POF组患者收缩期峰值流速(PSV)和舒张末期流速(EDV)均显著低于对照组(P<0.05),搏动指数(PI)和阻力指数(RI)均显著高于对照组(P<0.05)。结论 超声声像图特征对卵巢早衰的诊断具有较高的诊断预测准确度,可推广使用。Objective To investigate the diagnostic value of transvaginal color Doppler ultrasound in patients with premature ovarian failure and analyze its hemodynamic characteristics. Methods 102 patients with premature ovarian failure were treated as POF group, and 102 healthy women who underwent physical examination in our hospital at the same time were selected as control group by 1∶1 matching. The women in both groups received transvaginal ultrasound Doppler ultrasound, compared the characteristics of ultrasound images and hemodynamics, and the accuracy of ultrasound image characteristics in predicting the diagnosis of premature ovarian failure was analyzed by ROC curve. Results The duration of amenorrhea in POF group ranged from 6 to 19 months, with an average of(11.08±2.23) months. The ovarian volume(OV), follicle count(AFC), ovarian vascularization index(VI) and blood flow index(FI) in POF group were significantly lower than those in control group(P<0.05). The peak systolic velocity(PSV) and end diastolic velocity(EDV) in POF group were significantly lower than those in the control group(P<0.05), and the pulsatility index(PI) and resistance index(RI) were significantly higher than those in the control group(P<0.05). Conclusions Ultrasonographic features have a high accuracy in the diagnosis and prediction of premature ovarian failure and can be widely used.
