摘 要:目的 分析膝关节骨性关节炎(KOA)患者关节液中miR-34a-5p、miR-149-5p的表达及其临床意义。方法 收集2020年1月—2022年6月武汉市第一医院骨科就诊的KOA患者90例的临床资料,按X线片Kellgren-Lawrence(KL)标准分级分为轻度KOA组(1~2级患者)和重度KOA组(3~4级患者),各45例,另选取同期医院健康体检者45例作为健康对照组。利用qRT-PCR检测各组关节液中miR-34a-5p、miR-149-5p的表达量,通过ELISA法检测各组关节液中IL-1β的表达量,Spearman分析关节液中miR-34a-5p、miR-149-5p表达水平与IL-1β、KL分级的相关性,受试者工作特征曲线(ROC)分析关节液中miR-34a-5p、miR-149-5p表达对KOA严重程度的诊断价值。结果 膝关节液中miR-34a-5p、IL-1β表达水平比较,重度KOA组>轻度KOA组>健康对照组(F/P=19.21/<0.001、14.86/<0.001);膝关节液中miR-149-5p表达水平比较,重度KOA组<轻度KOA组<健康对照组(F/P=63.81/<0.001)。Spearman相关性分析显示,miR-34a-5p表达水平与IL-1β、KL分级呈显著正相关(r/P=0.835/<0.001、0.835/<0.001),miR-149-5p的表达水平与IL-1β、KL分级呈显著负相关(r/P=-0.828/<0.001、-0.943/<0.001)。ROC曲线分析显示,miR-34a-5p、miR-149-5p及二者联合预测KOA严重程度的曲线下面积分别为0.902、0.850、0.976,二者联合预测KOA严重程度的价值高于单项指标(Z/P=240.70/0.046,224.13/0.005)。结论 KOA患者关节液中miR-34a-5p、miR-149-5p的表达异常,且与KOA的临床分级和IL-1β显著相关,可作为诊断和监测KOA疾病活动的潜在标志物。Objective To analyze the expression and clinical significance of miR-34a-5p and miR-149-5p in joint fluid of patients with knee osteoarthritis(KOA).Methods The clinical data of 90 KOA patients from the Orthopedics Department of Wuhan First Hospital from January 2020 to June 2022 were collected.According to the Kellgren-Lawrence(KL)standard grading of X-ray film,45 patients were divided into mild KOA group(grade 1-2 patients)and severe KOA group(grade 3-4 patients).In addition,45 patients with physical examination in the same period were selected as the control group.The expression of miR-34a-5p and miR-149-5p in joint fluid of each group was detected by qRT-PCR,and the amount of expression IL-1βin joint fluid of each group was detected by ELISA.Spearman analyzed the correlation between expression level of miR-34a-5p,miR-149-5p and IL-1βin joint fluid,KL grading.The diagnostic value of the expression of miR-34a-5p and miR-149-5p in the joint fluid for the severity of KOA was analyzed by the working characteristic curve(ROC)of subjects.Results MiR-34a-5p,IL-1 in knee joint fluidβComparison of expression level,severe KOA group>mild KOA group>control group(F/P=19.21/<0.001,14.86/<0.001).Compared with the expression level of miR-149-5p in knee joint fluid,severe KOA group<mild KOA group<control group(F/P=63.81/<0.001).Spearman correlation analysis showed that the expression level of miR-34a-5p and IL-1β,KL grading showed significant positive correlation(r/P=0.835/<0.001,0.835/<0.001),and the expression level of miR-149-5p was correlated with IL-1β,KL grading showed significant negative correlation(r/P=-0.828/<0.001,-0.943/<0.001).The ROC curve analysis showed that the area under the curve of miR-34a-5p,miR-149-5p and their combined prediction of KOA severity were 0.902,0.850 and 0.976,respectively.The value of their combined prediction of KOA severity was higher than the single index(Z/P=240.70/0.046224.13/0.005).Conclusion The expression of miR-34a-5p and miR-149-5p in the joint fluid of KOA patients is abnormal
