中西医结合治疗神经性耳鸣的卫生经济学评价

摘　　要：目的采用成本-效果比的方法对中西医结合治疗神经性耳鸣进行卫生经济学评价。方法将神经性耳鸣辨证属邪郁少阳、痰瘀阻窍的患者120例采用随机数字表法分为西医常规组、臭氧组、中药组、综合组,各30例。4组患者分别给予基础西药、基础西药+臭氧疗法、基础西药+中药汤剂、基础西药+臭氧疗法+中药汤剂治疗。观察4组患者的临床疗效,并对4组进行成本-效果和敏感性分析。结果4组总有效率分别为56.7%、70.0%、73.3%、86.7%,综合组疗效最好(P<0.05);治疗后4组THI评分较治疗前均有明显改善(P<0.05),以综合组最佳(P<0.05);与西医常规组相比,每多增加1%的有效率,臭氧组所需费用增加294.98元,中药组所需费用增加84.59元,综合组费用增加160.17元。结论对于神经性耳鸣患者,可以通过适当增加医疗投入的方法以取得更好的临床疗效。Objective Health economics evaluation was used on the treatment of nervous tinnitus with integrated traditional Chinese and Western medicine by cost-effectiveness ratio method.Methods 120 patients with nervous tinnitus were randomly divided into western medicine routine group,three oxygen group,traditional Chinese medicine group and comprehensive treatment group,with 30 cases in each group.The patients in the four groups were treated with western medicine routine treatment,western medicine routine treatment+triple oxygen autohemotherapy,western medicine routine treatment+Qingre-erming formula,western medicine routine treatment+triple oxygen autohemotherapy+Qingre-erming formula.The clinical efficacy of the four groups was observed,and the cost-effectiveness and sensitivity were evaluated.Results The total effective rates of the four groups were 56.7%,70.0%,73.3%and 86.7%respectively.The curative effect of the comprehensive treatment group was the best(P<0.05);and the THI score of the four groups were significantly improved after treatment compared with those before treatment(P<0.05),the comprehensive treatment group was the best(P<0.05).Compared with the conventional western medicine group,for every 1%increase in the effective rate,the cost of the three oxygen group increased by 294.98 yuan,the cost of the traditional Chinese medicine group increased by 84.59 yuan,and the cost of the comprehensive treatment group increased by 160.17 yuan.Conclusion For patients with nervous tinnitus,we can appropriately increase medical investment to achieve better clinical efficacy.
