独活寄生汤加减联合胫骨高位截骨术对早中期膝骨性关节炎的治疗作用

摘　　要：目的探讨独活寄生汤联合胫骨高位截骨术对早中期膝关节骨性关节炎(KOA)的治疗作用。方法选取KOA患者30例(30膝),均为单膝。采用随机数字表法,将其分为对照组和实验组,每组15例。两组患者入院后完善检查并排除手术禁忌证,对照组患者行关节镜下膝关节清理胫骨高位截骨术(HTO)治疗,实验组患者给予独活寄生汤联合关节镜下膝关节清理HTO治疗,术后均给予膝关节功能锻炼。对比两组患者术前及术后1个月、2个月视觉模拟评分(VAS)、膝关节功能评分(WOMAC)、医院膝关节评分(HSS)。结果术后1个月、2个月实验组VAS,WOMAC与对照组比较,差异有统计学意义(P<0.05),HSS高于对照组,组间比较差异有统计学意义(P<0.05)。结论独活寄生汤联合HTO治疗早中期KOA,能够明显减轻患者术后疼痛,有利于患者膝关节功能更好恢复。Objective To investigate the therapeutic effect of Duhuo Jiji decoction combined with high tibial osteotomy on knee osteoarthritis(KOA)in the early and middle stages.Methods Thirty patients with KOA(30 knees)admitted to our department were selected,and all of them were single knee.Using random number table method,they were divided into the control group and the experimental group,with 15 cases in each group.After admission,two groups of patients were thoroughly examined and excluded surgical contraindications.The patients in the control group were treated with arthroscopic high tibial debridement of the knee joint,and the patients in the experimental group were treated with Duhuo Jiji decoction combined with arthroscopic knee joint debridement and high tibial osteotomy,and postoperative knee function exercises were given.The visual analogue scale(VAS),the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)and hospital for special surgery score(HSS)were compared between two groups before surgery and at one month and two months after surgery.Results One month and two months after operation,the VAS and WOMAC of the experimental group were significantly different from those of the control group(P<0.05),and the HSS was higher than that of the control group which was statistically significant(P<0.05).Conclusion Duhuo Jiji decoction combined with high tibial osteotomy in the treatment of KOA in the early and middle stages can significantly reduce postoperative pain and facilitate the recovery of knee joint function.
