固定平台外侧单髁关节置换术治疗膝外侧间室骨关节炎的早期临床疗效分析

摘　　要：目的探讨固定平台外侧单髁关节置换术(UKA)治疗膝关节外侧间室骨关节炎的临床疗效。方法回顾性分析2018年6月~2022年6月收治的外侧骨关节炎患者18例,其中2例患者术中改行全膝关节置换术(TKA),共16例患者行固定平台外侧UKA手术。患者年龄(61.19±5.37)岁,随访时间(25.56±8.81)个月。使用膝关节KSS评分系统和影像学平片评估手术效果。结果16例患者均获得随访,KSS疼痛评分从术前的(17.19±2.56)分提高到最终随访时的(41.25±7.64)分(P<0.01);KSS膝关节评分和功能评分分别从术前的(61.88±3.18)分和(52.81±10.32)分提高至随访时的(87.13±8.92)分和(88.13±5.12)分(P<0.01);膝关节的活动度从术前的(119.00±7.33)°恢复到最终随访时的(124.63±2.45)°;胫股角从术前外翻(6.29±0.88)°变为最终随访时的外翻(3.59±0.83)°。根据KSS膝关节评分分类的总体结果为:优秀15例,一般1例。结论外侧单髁置换术早期在改善疼痛、提高膝关节评分、功能评分和恢复膝关节运动方面均令人满意。Objective To investigate the clinical results of fixed platform lateral unicompartmental knee arthroplasty(UKA)in the treatment of lateral unicompartmental osteoarthritis.Methods From June 2018 to June 2022,the retrospective investigation was performed on 18 patients with lateral unicompartmental osteoarthritis who were treated in the hospital.Among them,two patients was converted to total knee arthroplasty(TKA)and sixteen patients underwent lateral UKA surgery with a fixed platform.The age of the patients was(61.19±5.37)years,and the follow-up time was(25.56±8.81)months.The clinical and radiographic results with use of the Knee Society(KSS)clinical rating system and plain radiography.Results All 16 patients were followed-up.The KSS pain score improved from(17.19±2.56)preoperatively to(41.25±7.64)at the final follow-up(P<0.01);The KSS knee joint score and functional score significantly increased from(61.88±3.18)and(52.81±10.32)to(87.13±8.92)and(88.13±5.12)(P<0.01);The range of motion of the knee joint recovered from(119.00±7.33)°to(124.63±2.45)°at the final follow-up;The tibiofemoral valgus angle changed from(6.29±0.88)°to(3.59±0.83)°at the final follow-up.The overall results classified based on the KSS knee score were"excellent"in 15 cases and"fair"in one case.Conclusion It is satisfactory for lateral UKA in improving pain,knee joint score,function score and knee joint movement in the early stage.
