犬异位性皮炎的中兽医诊疗

摘　　要：犬异位性皮炎(CAD)临床发病率较高,引起犬长期剧烈瘙痒。西医治疗以外用糖皮质激素和免疫调节剂为主,停药后易反复,甚至加重。一例反复发作的犬异位性皮炎病例,患犬全身皮肤多处病灶,瘙痒剧烈,频繁挠舔,毛焦无光泽,皮温高且皮肤大面积泛红,舌质红绛,苔薄、较干,喜凉,性情暴躁。中兽医辨证为阴虚内热证且有脾虚之象;卫表不固,风邪犯表;肝失所养,肝气郁滞,郁而化热。从整体入手,用中药内治法整体调节患犬的过敏性体质,以治其阴虚内热证为主,方内诸药通过相须相使等发挥补气健脾、清热泻火、祛风利湿、解毒止痒等功效。患犬共就诊7次,治疗过程中根据患犬具体情况随证调方,共内服中药2个月,患犬皮肤症状得到明显改善,被毛柔软发亮,复发次数减少,未出现副作用。Canine atopic dermatitis( CAD) has a high clinical incidence and causes prolonged and intense pruritus in dogs. Western medicine treatment is based on external glucocorticoids and immunomodulators,which have large side effects and are prone to recurrence and even aggravation after discontinuation of medication. In this case,a case of CAD with itching,oozing and other skin symptoms and recurrent attacks was treated with traditional Chinese veterinary medicine. In this case,the dog had a red and vivid tongue,thin and dry coating with lines,scorched and lusterless fur,high skin temperature and extensive reddening of the skin. At the same time,the dog had a long duration of skin disease,allergen test results showed a variety of food allergies,and could only eat homemade dog food,with signs of spleen deficiency. Multiple skin lesions all over the body,intense itching and frequent scratching in the affected dog,thus the body surface was not solid,and the wind evil offended the surface. The dog had a long and recurring illness,and was in a poor mood,with the liver losing its nourishment and the liver Qi becoming stagnant and turning heat into depression. In this case,we used Chinese herbal medicine to regulate the allergic constitution of the affected dog,mainly treating its Yin deficiency and internal heat,nourishing Yin and nourishing blood,clearing heat and fire,strengthening the spleen and dispelling dampness,dispelling wind and relieving itching. The dog in this case was consulted 7 times,the prescription was adjusted according to the specific conditions of the dog at the time of the consultation,the herbs were taken internally for a total of 2 months. After the treatment,the dog’s skin symptoms improved significantly,the hair became soft and shiny,recurrence was reduced and no side effects occurred during the treatment. This case is summarized to provide a basis and ideas for the treatment of similar diseases in Chinese veterinary medicine.
