阿莫西林克拉维酸钾致脓疱型红皮病1例及药学监护

摘　　要：目的分析阿莫西林克拉维酸钾分散片引起脓疱性红皮病的一般规律及注意事项。方法回顾性分析1例使用阿莫西林克拉维酸钾分散片致脓疱性红皮病的临床病例。结果经过甲泼尼龙静脉注射抗过敏、抗炎治疗三天后逐渐减量至口服;抗感染予美罗培南加万古霉素抗感染治疗;氯雷他定抗组胺治疗和维生素C减轻血管渗透等对症支持治疗,患者皮疹好转出院。结论临床应用阿莫西林克拉维酸钾口服制剂时,也应密切观察,以防止过敏反应发生。对有过敏体质者,尤其是合并多种基础疾病的老年患者应慎用,如发生不良反应立即停药,且及时对症治疗。OBJECTIVE To summarize the general rule and matters needing attention of pustular erythroderma caused by amoxicillin clavulanate potassium dispersible tablets.METHODS A case of pustular erythroderma caused by amoxicillin clavulanate potassium dispersible tablets was analyzed retrospectively.RESULTS After three days of intravenous injection of methylprednisolone for anti-allergic and anti-inflammatory treatment,the dose was gradually reduced to oral administration;anti-infective treatment with meropenem plus vancomycin;loratadine antihistamine treatment and vitamin C to reduce vascular infiltration and other symptomatic support therapy,the patient′s skin rash improved and discharged from the hospital.CONCLUSION The clinical application of amoxicillin clavulanate potassium oral preparation should also be closely observed to prevent allergic reactions.For people with allergic constitution,especially the elderly patients with a variety of underlying diseases should be used with caution,such as adverse reactions immediately stop the drug,and immediate symptomatic treatment.
