抗结核药物性肝损伤患者血清细胞角蛋白18-M30与M65的临床意义的探讨

摘　　要：目的探讨抗结核药物性肝损伤(ATB-DILI)患者血清细胞角蛋白18-M30与M65的临床意义。方法收集2017年~2022年在我院诊断ATB-DILI的患者70例和37例健康人。采用ELISA检测血清CK18-M30、CK18-M65水平,收集其生化学及凝血检验指标。结果ATB-DILI组与健康人群在CK18-M30对数、CK18-M65对数、ALB、TBIL、DBIL、IBIL、ALT、AST、GGT、ALP、INR有统计学差异(P<0.05)。Pearson相关分析,CK18-M30对数、CK18-M65对数与ALB正相关,与INR负相关(P<0.05),M30/M65与ALT正相关(P<0.05)。Spearman等级相关分析,ATB-DILI严重程度与CK18-M30对数、CK18-M65对数、ALB负相关(P<0.05),与年龄、TBIL、DBIL、IBIL、ALT、AST、GGT、ALP、INR正相关(P<0.05)。结论CK18-M30、M65对ATB-DILI的有一定的临床意义,可用于疾病严重程度的判断。OBJECTIVE To investigate the clinical significance of serum keratin 18-M30 and M65 in patients with anti-tuberculous drug-induced liver injury(ATB-DILI).METHODS Seventy patients with anti-tuberculous drug-induced liver injury diagnosed in our hospital from 2017 to 2022 were collected,compared with 37 healthy people.Serum levels of CK18-M30 and CK18-M65 were detected by ELISA,and biochemical and coagulation indexes were collected.RESULTS There were statistically significant in CK18-M30 logarithm,CK18-M65 logarithm,ALB,TBIL,DBIL,IBIL,ALT between the group of ATB-DILI and the healthy population(P<0.05).Pearson correlation analysis showed that CK18-M30 logarithm and CK18-M65 logarithm were positively correlated with ALB(P<0.05),and negatively correlated with INR(P<0.05).M30/M65 was positively correlated with ALT(P<0.05).Spearman rank correlation analysis showed that the severity of ATB-DILI was negatively correlated with CK18-M30 logarithm,CK18-M65 logarithm and ALB(P<0.05).They were positively correlated with age,TBIL,DBIL,IBIL,ALT,AST,GGT,ALP and INR(P<0.05).CONCLUSION CK18-M30 and M65 have certain clinical significance in ATB-DILI,and can be used to judge the severity of the disease.
