摘 要:目的 观察冠状动脉(冠脉)造影术后循环微小RNA-188-5p(miR-188-5p)的变化,探讨其在对比剂肾病(CIN)发生中的预测价值。方法 从GEO数据库获得GSE81400和GSE130796表达矩阵并分析miR-188-5p在CIN大鼠血浆及肾脏中的表达;选取2020年6月至2020年12月于南京医科大学第一附属医院心血管内科行冠脉造影术后并发CIN患者作为CIN组,另按1:3比例纳入同时期无并发CIN的患者作为对照组,实时荧光定量PCR检测冠脉造影术前及术后12 h患者循环miR-188-5p表达,分析其临床意义。结果 分析GEO数据库中数据集显示CIN大鼠血浆中(1.116±0.034 vs. 1.00±0.031,P=0.005)及肾脏中[(GSE81400:1.860±0.309 vs. 1.00±0.051),P=0.003;(GSE130796:2.360±0.317 vs. 1.00±0.497),P=0.031]的miR-188-5p表达高于对照组;共纳入CIN患者42例及非CIN患者126例,两组患者性别、血肌酐(SCr)值、血尿素氮(BUN)、药物使用情况及miR-188-5p表达量无显著差异(P均>0.05)。但两组患者在年龄(67.7±9.7 vs. 64.2±9.5,P=0.042)、合并糖尿病(52.4%vs. 26.2%,P=0.002)及贫血(21.4%vs. 9.5%,P=0.043)、造影剂用量(126.4±28.6 vs. 117.1±21.8,P=0.030)方面存在差异;造影剂应用12 h后,CIN组患者循环miR-188-5p表达水平高于对照组(1.852±0.890 vs.1.266±0.537,P<0.001);单因素及多因素logistic回归分析显示年龄(OR=1.016,P=0.033)、糖尿病(OR=3.582,P=0.004)、造影剂用量(OR=1.026,P=0.004)及术后miR-188-5p表达量(OR=2.835,P=0.001)与发生CIN密切相关。结论 循环miR-188-5p具有作为CIN预测标志物的潜力。Objective To observe the changes of circulating microRNA-188-5p(miR-188-5p) after coronary angiography(CAG), and discuss the predictive value of miR-188-5p to contrast-induced nephropathy(CIN).Methods The expression matrix of GSE81400 and GSE130796 was obtained from GEO database, and expressions of miR-188-5p in plasma and kidney were analyzed in CIN rats. The patients with CIN after CAG were chosen into CIN group and patients without CIN were chosen, at a ratio of 1:3, into control group from Department of Cardiology in the First Affiliated Hospital of Nanjing Medical University from June 2020 to Dec. 2020. The expression of miR-188-5p was detected by using real-time fluorescence quantitative polymerase chain reaction(RT-qPCR) before and 12 h after CAG, and its clinical significance was analyzed. Results The results of the data set in GEO database showed that miR-188-5p expressions in plasma(1.116±0.034 vs. 1.00±0.031, P=0.005) and kidney(GSE81400:1.860±0.309 vs. 1.00±0.051, P=0.003;GSE130796: 2.360±0.317 vs. 1.00±0.497, P=0.031) were higher in CIN rats than those in control rats. There were totally 42 CIN patients(CIN group) and 126 non-CIN patients(control gruop) included. There were no significant differences in gender, serum creatinine(SCr), blood urea nitrogen(BUN),drug usage and miR-188-5p expression between 2 groups(all P>0.05). There were differences in age(67.7±9.7 vs. 64.2±9.5, P=0.042), complicated diabetes(52.4% vs. 26.2%, P=0.002), animia(21.4% vs. 9.5%, P=0.043)and dose of contrast(126.4±28.6 vs. 117.1±21.8, P=0.030) between 2 groups. After using contrast for 12 h, miR-188-5p expression was higher in CIN group than that in control group(1.852±0.890 vs. 1.266±0.537, P<0.001).The results of multi-factor Logistic regression analysis showed that age(OR=1.016, P=0.033), diabetes(OR=3.582,P=0.004), dose of contrast(OR=1.026, P=0.004) and postoperative miR-188-5p expression(OR=2.835, P=0.001)were closely correlated to CIN occurrence.
