政策工具视角下我国医养结合服务发展的影响组态分析

摘　　要：目的:基于对政策工具如何影响医养结合服务发展的分析,探究以政策工具推动医养结合服务发展的可行路径,为我国不同地区医养结合服务体系的建设工作提供决策参考。方法:选取我国十大试点城市为案例,建立分析框架及理论模型,运用模糊集定性比较分析,揭示政策工具子维度对当地医养结合服务发展的影响组态。结果:政策工具视角下医养结合服务发展的条件组合可以概括为供给驱动需求协同型、供给驱动环境协同型和多维度协同型3种模式,此3种影响医养结合服务发展的条件构型分别能够覆盖16.75%、41.50%、25.00%的案例。结论:政策工具视角下影响医养结合服务发展的因素呈现多样化,需要切实结合信息支持、目标规划、资金投入等政策工具和当地客观情况,拓展服务发展动力来源和路径,提升资源配置和整体服务能力,转变政策重点和管理体制机制。Objective:Based on analyzing how the policy tools influence the development of integrated medical treatment and care service for older adults, this study explored the feasible path to promote the service development with policy tools, which aimed at providing decision-making references for the construction of integrated medical treatment and care service systems for older adults across different regions of China. Methods:This study selected ten pilot cities in China as cases, established an analytical framework and theoretical model, and used fuzzy set qualitative comparative analysis to reveal the influence configuration of sub-dimensions of policy tools on the development of integrated medical treatment and care service for older adults. Results:From the perspective of policy tools, the combination of conditions for the development of integrated medical treatment and care service for older adults could be summarized as three models: supply-driven demand synergy, supply-driven environment synergy and multi-dimensional synergy, and the three conditional configurations that affected the service development could cover 16.75%, 41.50% and 25.00% of the cases respectively. Conclusion:In view of policy tools, the factors affecting the development of integrated medical treatment and care service for older adults are diversified. It is necessary to effectively combine policy tools such as information support, target planning, and capital investment and local objective conditions, to expand the power sources and paths of service development, improve the resource allocation and overall service capabilities, and change the policy priorities and management systems and mechanisms.
