结膜松弛症中医证型、证素与眼部症状的关联规则分析

摘　　要：目的探索结膜松弛症(CCh)中医证型-证素-症状之间的关联性。方法纳入2019年2月—2020年1月上海中医药大学附属普陀医院诊断为CCh的患者250例,均以右眼纳入研究。制作CCh临床流行病学调查表,收集患者一般信息及眼部症状,由2名中医专家现场进行中医证型判定。利用“便携式中医智能镜”收集患者中医证素。将采集到的临床数据录入IBM SPSS Modeler 18.0进行预处理,形成“数据源-类型-Apriori-网络”数据流,再利用统计软件进行关联规则(AR)统计分析。结果(1)一般情况:常见的中医证型有5个,分别为肝肾阴虚证、肺阴不足证、气阴两虚证、脾虚气弱证、阴虚湿热证。中医证素有5个,分别为气虚、阴虚、气滞、阳虚、痰湿。眼部症状共8个,分别为异物感、畏光、流泪、眼痒、疼痛、干涩、视疲劳、视物模糊。(2)中医证型与证素的AR分析:关联最强的前5位分别是气阴两虚-阴虚、肝肾阴虚-阴虚、气阴两虚-气虚、脾虚气弱-气虚、肝肾阴虚-气虚。(3)中医证型与眼部症状的AR分析:关联最强的前5位分别是脾虚气弱-流泪、肝肾阴虚-流泪、气阴两虚-视物模糊、气阴两虚-异物感、脾虚气弱-异物感。(4)中医证素与眼部症状的AR分析:关联最强的前5位分别是阳虚-流泪、阴虚-流泪、气虚-流泪、痰湿-视物模糊、气虚-视物模糊。结论CCh的中医证型、证素、眼部症状之间存在较强的关联性,有助于临床医生对CCh的诊断和辨证。OBJECTIVE To explore the correlation between Traditional Chinese Medicine(TCM)syndrome types,symptom elements and ocular symptoms of conjunctivochalasis(CCh).METHODS A total of 250 patients diagnosed as CCh in Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from February 2019 to January 2020 were included in the study,whose right eyes were included in the study.CCh epidemiology questionnaire was used to collect general information and ocular symptoms of the patients,and two TCM experts were responsible for on-site TCM symptom type determination.The"portable TCM smart mirror"was used to collect TCM syndrome elements of the patients.The collected clinical data was entered into IBM SPSS Modeler 18.0 for preprocessing to form a"data source-type-Apriori-network"data stream,and then statistical software was used for association rule(AR)statistical analysis.RESULTS(1)General information:There were five common TCM syndrome types,including liver and kidney Yin deficiency syndrome,lung Yin deficiency syndrome,Qi and Yin deficiency syndrome,spleen and Qi deficiency syndrome,as well as Yin deficiency and damp-heat syndrome.There were five TCM syndrome elements,including Qi deficiency,Yin deficiency,Qi stagnation,Yang deficiency and phlegm dampness.There were eight ocular symptoms,including foreign body sensation,photophobia,tearing,eye itching,pain,dryness,visual fatigue and blurred vision.(2)AR analysis of TCM syndrome types and syndrome elements:The top five pairs with the strongest correlations were Qi and Yin deficiency-Yin deficiency,liver and kidney Yin deficiency-Yin deficiency,Qi and Yin deficiency-Qi deficiency,spleen deficiency-Qi deficiency,liver and kidney Yin deficiency-Qi deficiency.(3)AR analysis of TCM syndrome types and ocular symptoms of CCh:The top five pairs with the strongest correlations were spleen deficiency and Qi deficiency-tearing,liver and kidney Yin deficiency-tearing,Yin deficiency and Qi deficiency-blurred vision,Yin deficiency and Qi deficiency-foreign body sen
