摘 要:目的探究艾灸结合肺纤通络方对博莱霉素(BLM)所致肺纤维化(PF)大鼠的治疗效果及对NF-κB/TGF-β_(1)/smad 3通路的调控作用。方法从50只大鼠中随机选取10只为对照组,其余40只采用博莱霉素诱导建立PF模型,4只大鼠死亡,其余随机分为PF组、艾灸组、联合组及阳性组,各9只。艾灸组采用艾灸治疗,联合组在艾灸的穴位处滴加5滴肺纤通络方并艾灸至干燥,阳性组给予泼尼松3 mL·kg^(-1),PF组及对照组给予等剂量的生理盐水。连续给药30d后,测量各组血清炎症因子及氧化应激指标水平,观察肺组织病理学变化,检测NF-κB/TGF-β_(1)/smad 3通路蛋白表达情况。结果与对照组比较,PF组血清IL-1β、IL-6、TNF-α水平及HYP、GSH水平升高,MDA水平降低,NF-κB、TGF-β_(1)、smad 3 mRNA水平升高,NF-κB、TGF-β_(1)、smad 3蛋白表达上调(P<0.05);与PF组比较,艾灸组、联合组及阳性组血清IL-1β、IL-6、TNF-α水平及HYP、GSH水平降低,MDA水平升高,NF-κB、TGF-β_(1)、smad 3 mRNA水平降低,NF-κB、TGF-β_(1)、smad 3蛋白表达下调(P<0.05);与艾灸组比较,联合组及阳性组血清IL-1β、IL-6、TNF-α及HYP、GSH水平降低,MDA水平升高,NF-κB、TGF-β_(1)、smad 3 mRNA水平降低,NF-κB、TGF-β_(1)、smad 3蛋白表达下调(P<0.05);与联合组比较,阳性组血清IL-1β、IL-6、TNF-α水平及HYP、GSH水平降低,MDA水平升高,NF-κB、TGF-β_(1)、smad 3 mRNA水平降低,NF-κB、TGF-β_(1)、smad 3蛋白表达下调(P<0.05)。结论艾灸结合肺纤通络方能够改善BLM所致PF大鼠炎症指标及氧化应激指标,对肺组织起到保护作用,其作用可能与NF-κB/TGF-β_(1)/smad 3通路有关。Objective To investigate the therapeutic effect of moxibustion combined with Feixian Tongluo decoction on bleomycin(BLM)induced pulmonary fibrosis(PF)in rats and its regulatory effect on the NF-κB/TGF-β_(1)/smad 3 pathway.Methods Ten of 50rats were randomly selected as the control group,and the other 40 rats were induced by bleomycin to establish a PF model,among which 4 rats died,and the rest were randomly divided into a PF group,a moxibustion group,a combination group and a positive group,with 9 rats in each group.The moxibustion group was treated with moxibustion.In the combination group,5 drops of Feixian Tongluo decoction were administered to the acupoints where moxibustion was carried out until they got dry.The positive group was treated with prednisone 3mL·kg^(-1),and the PF group and the control group were given the equal dose of normal saline.After 30 days of continuous administration,the levels of serum inflammatory factors and oxidative stress indexes were measured,the pathological changes of lung tissues were observed,and the protein expression of the NF-κB/TGF-β_(1)/smad 3 pathway was detected.Results Compared with the control group,the serum levels of IL-1β,IL-6,TNF-α,the levels of HYP and GSH were increased in the PF group,while the level of MDA was decreased,and the mRNA levels of NF-κB,TGF-β_(1)and smad 3 were increased,the protein expression levels of NF-κB,TGF-β_(1)and smad 3 were up-regulated(P<0.05).Compared with the PF group,in the moxibustion group,the combination group and the positive group,the serum levels of IL-1β,IL-6 and TNF-α,the levels of HYP and GSH were decreased,the level of MDA was increased,and the mRNA levels of NF-κB,TGF-β_(1)and smad 3 were decreased,while the protein expression levels of NF-κB,TGF-β_(1)and smad 3 were down-regulated(P<0.05).Compared with the moxibustion group,in the combination group and the positive group,the serum levels of IL-1β,IL-6,TNF-α,the levels of HYP and GSH were decreased,the level of MDA was increased,the mRNA levels of NF-κB
