摘  要:目的 探究子宫内膜息肉组织中乙醛脱氢酶1(ALDH1)和环氧合酶-2(COX-2)的表达及其与术后复发的关系。方法 筛选82例子宫内膜息肉患者作为研究对象,收集并整理所有受试者的病例资料,以免疫组化法完成子宫内膜息肉组织中ALDH1和COX-2的表达检测,根据患者子宫内膜息肉患者术后6个月内复发情况将其分为复发组和正常组。Logistic多因素回归分析探讨影响子宫内膜息肉患者术后复发的相关因素,以受试者工作特征曲线(ROC)评估子宫内膜息肉组织中ALDH1和COX-2的表达对子宫内膜息肉患者术后复发的预测价值。结果 82例子宫内膜息肉患者中术后复发21例,复发率25.61%。复发组BMI≥24 kg·m^(-2)、合并妇科疾病、ALDH1阳性表达、COX-2阳性表达所占比例均高于正常组(P <0.05)。Logistic多因素回归分析结果显示,BMI≥24 kg·m^(-2)、合并妇科疾病、ALDH1阳性表达、COX-2阳性表达均为子宫内膜息肉患者术后复发的影响因素(P <0.05)。ROC分析结果显示,子宫内膜息肉组织中ALDH1和COX-2表达单一及联合对子宫内膜息肉患者术后复发ROC预测的曲线下面积(AUC)分别为0.763、0.659、0.864,且单一预测效能低于联合(P <0.05)。结论 子宫内膜息肉组织中ALDH1和COX-2表达可用于预测子宫内膜息肉患者术后复发,且联合预测效能较高。Objective To explore the expressions of acetaldehyde dehydrogenase 1(ALDH1) and cyclooxygenase-2(COX-2) in endometrial polyps and their relationship with the postoperative recurrence of endometrial polyps. Methods A total of 82 patients with endometrial polyps admitted to our hospital from July 2019 to June 2021 were selected as the study subjects, and their case data were collected and sorted out. The expressions of ALDH1 and COX-2 in the endometrial polyp tissues were detected by immunohistochemistry. They were divided into the recurrence group and the normal group according to the recurrence of endometrial polyps within 6 months after operation. A Logistic multivariate regression analysis was used to explore the related factors that affected their recurrence after operation. The receiver operating characteristic curve(ROC) was used to evaluate the predictive value of the expressions of ALDH1 and COX-2 in endometrial polyps for their postoperative recurrence. Results Among the 82 patients with endometrial polyps, 21 recurred after operation, with a recurrence rate at 25.61%. The proportions of those with BMI ≥ 24 kg·m^(-2), combined gynecological diseases, ALDH1 positive expression, and COX-2 positive expression in the recurrence group were higher than those in the normal group(P<0.05). The Logistic multivariate regression analysis showed that BMI ≥ 24 kg·m^(-2), combined gynecological diseases, ALDH1 positive expression and COX-2 positive expression were all the influencing factors for the postoperative recurrence of endometrial polyps(P<0.05). The ROC analysis results showed that the areas under the curve(AUC) of single and combined expressions of ALDH1 and COX-2 in the endometrial polyp tissues for predicting the postoperative recurrence of endometrial polyps in patients with endometrial polyps were 0.763, 0.659 and 0.864,respectively, and the single prediction efficiency was lower than that of the combination(P<0.05).
