基于“证”转归论肿瘤转移器官偏嗜的中医发生学

摘　　要：肿瘤转移器官偏嗜性直接影响肿瘤临床疗效和预后,对转移器官偏嗜性发生机制进行剖析是当前癌症研究的新兴课题。中医认为,肝、肺二脏是消化系统恶性肿瘤转移偏嗜的主要器官,且二脏互为靶器官;脑部气血汇聚,极易成为肿瘤转移的靶器官;肺朝百脉,通过血脉传舍的肿瘤细胞易到达肺脏并在此生长;肝藏血,其内血行缓慢,癌毒稽留,易成瘀血;淋巴系统是胃癌远处转移偏嗜的主要途径。虽然中医对肿瘤转移偏嗜性的认知尚未成体系,但基于六经传变、脏腑生克、卫气营血等中医“证”转归的理论,对探索肿瘤转移器官偏嗜发生机制极具启发性。Metastatic organ preference of tumor directly affects the clinical efficacy and prognosis of tumor, and the analysis of the mechanism of metastatic organ preference is a new topic in current cancer research.In traditional Chinese medicine it is believed that the Liver and Lung are the main Zang of metastatic preference of digestive malignant tumors, and the two are the target organs of each other.Qi and blood converge in brain, which is easy to become the target organ of tumor metastasis.The lung is in the direction of hundreds of meridians, and the tumor cells passing through the meridians are easy to reach the Lung and grow here.The Liver stores blood, and the slow blood flow in it makes it easy for the cancer toxin being retained and inducing blood stasis;Lymphatic system is the main pathway of distant metastasis of gastric cancer.Although in traditional Chinese medicine the cognition of tumor metastasis preference has not yet established systematically, the theory of TCM "syndrome" conversion based on the Six Meridians, the Sheng and Ke relationship between Zang and Fu as well as Wei Qi and Ying Xue Theory is very enlightening for exploring the mechanism of tumor metastasis organ preference.
