基于“脾滞当启脾”思想论治儿童单纯性肥胖症

摘　　要：儿童单纯性肥胖症多呈虚实夹杂之征象,病位主要在脾胃,脾胃升降功能失司是胃强脾滞证的病机关键。立足小儿生理病理特点,结合仝小林教授“脾滞当启脾”思想,临证时当以运脾导滞、抑强扶弱为主要治疗原则。通过运脾导滞以助脾胃升降有度,膏消浊降。方以半夏泻心汤加减治疗,辛开苦降以散结除痞、调畅气机,和胃降逆以化痰降浊、消膏降脂,加砂仁、大腹皮、党参、甘草,以补运同举,标本兼顾。Children′s simple obesity often presents the symptoms of deficiency and excess at the same time, and the disease location is mainly in the Spleen and Stomach.The loss of the ascending and descending function of the Spleen and Stomach Qi is the key to the pathogenesis of the syndrome of strong stomach with Spleen stagnation.Based on the physiological and pathological characteristics of children, combined with Professor Tong Xiaolin′s thought of "removing Spleen stagnation",the main treatment principle is to promote the Spleen function to remove the stagnation by suppressing the strong and strengthening the weak.It can help the Spleen and Stomach Qi to move up and down to a certain degree by activating the Spleen and removing the stagnation, so as to eliminate fat and reduce turbidity.The prescription used is the modified Banxia Xiexin Decoction, which is to relieve pain, remove the swelling, regulate Qi, reduce the Phlegm and Turbidity, eliminate the turbidity and reduce the fat.Medicinals such as Sharen(villosum),Dafupi(codonopsis pilosula) Dangshen(codonopsis pilosula) and Gancao(licorice) can be added to the prescription to supplement and motivate function of Spleen and Stomach, to achieve balanced treatment of symptoms and syndromes.
