摘 要:目的:探讨通窍活血汤对创伤性脑损伤大鼠高迁移率族蛋白B1(high mobility group protein B1,HMGB1)、白细胞介素-6(interleukin-6,IL-6)的影响。方法:将90只18周龄SPF级SD大鼠随机分为空白组(n=15)和造模组(n=75),空白组大鼠不予任何处理,造模组大鼠采用改良的Feeney′s重物自由落体撞击法建立创伤性脑损伤大鼠模型,造模完成后将造模组大鼠随机分为模型组、依达拉奉组及通窍活血汤高(9 g·mL^(-1))、中(4.5 g·mL^(-1))、低(2.25 g·mL^(-1))剂量组,每组15只。通窍活血汤高、中、低剂量组大鼠灌胃给予相应剂量的通窍活血汤溶液,依达拉奉组大鼠灌胃给予依达拉奉注射液,空白组和模型组大鼠灌胃给予生理盐水,灌胃体积为10 mL·kg^(-1),每天2次,连续干预7天。使用改良的Garcia JH(mGarcia JH)评分评估创伤性脑损伤后大鼠的行为变化及神经功能损伤程度;ELISA法检测脑组织HMGB1、IL-6的水平;TUNEL染色检测大脑皮质神经细胞凋亡情况;HE染色检测脑组织病理学变化;湿干比法测定脑组织含水量。结果:与空白组比较,模型组大鼠mGarcia JH评分显著降低(P<0.05),HMGB1、IL-6的水平及神经凋亡细胞数、脑组织含水量显著升高(P<0.05);与模型组比较,通窍活血汤组大鼠mGarcia JH评分显著升高(P<0.05);HMGB1、IL-6的水平及神经凋亡细胞数、脑组织含水量显著降低(P<0.05)。HE染色显示,模型组大鼠脑组织血肿周围呈充血肿胀、出血,细胞坏死,有胶质细胞增生;通窍活血汤中剂量组大鼠脑组织病理损伤明显改善。结论:通窍活血汤可以降低HMGB1、IL-6的含量,减少神经细胞凋亡和脑组织含水量,促进创伤性脑损伤的神经恢复。Objective: To explore the effect of Tongqiao Huoxue Decoction on high mobility group protein B1(HMGB1) and interleukin-6(IL-6) in rats with traumatic brain injury.Methods: 90 18-week-old SPF grade SD rats were randomly divided into the blank group(n=15) and the model group(n=75).The blank group rats were not treated.The model group rats were established with the modified Feeney′s weight free fall impact method.After the model was completed, the model group rats were randomly divided into the model group, the edaravone group and the high(9 g·mL^(-1)),medium(4.5 g·mL^(-1)),and low(2.25 g·mL^(-1)) dose groups of Tongqiao Huoxue Decoction, 15 in each group.The rats in the high, middle and low dose groups of Tongqiao Huoxue Decoction were given Tongqiao Huoxue Decoction solution by gavage, the edaravone group was given edaravone injection by gavage, the blank group and the model group were given physiological saline by gavage, 2 mL each time, twice a day, for 7 consecutive days.The modified Garcia JH(mGarcia JH) score was used to evaluate the behavioral changes and the degree of neurological damage of rats after traumatic brain injury;The levels of HMGB1 and IL-6 in brain tissue were detected by ELISA;TUNEL staining was used to detect the apoptosis of neurons in the cerebral cortex;HE staining was used to detect the pathological changes of brain tissue;The water content of brain tissue was determined by wet/dry ratio method.Results: Compared with the blank group, the mGarcia JH score of the model group was significantly lower(P<0.05),and the levels of HMGB1 and IL-6,the number of apoptotic cells and the water content of brain tissue were significantly higher(P<0.05).Compared with the model group, the mGarcia JH score of rats in the Tongqiao Huoxue Decoction group was significantly higher(P<0.05).The levels of HMGB1 and IL-6,the number of apoptotic cells and the water content of brain tissue were significantly decreased(P<0.05).HE staining indicates that there were hyperemia, swelling, hemorrhage, cell necrosis an
