中医正骨联合理筋手法治疗儿童寰枢关节半脱位的临床研究

摘　　要：目的观察中医正骨联合理筋手法治疗儿童寰枢关节半脱位的临床疗效。方法将64例寰枢关节半脱位患儿按照随机数字表法分为2组,对照组32例予颈部理筋手法治疗,治疗组32例在对照组基础上联合中医正骨手法治疗。2组均治疗3周后统计疗效,比较2组治疗前后寰齿侧间距差值及寰齿前间距变化情况。结果与本组治疗前比较,2组治疗后寰齿侧间距差值均减小(P<0.05),且治疗组治疗后寰齿侧间距差值小于对照组(P<0.05)。与本组治疗前比较,2组治疗后寰齿前间距均减小(P<0.05),且治疗组治疗后寰齿前间距小于对照组(P<0.05)。治疗组总有效率97%(31/32),对照组总有效率75%(24/32),治疗组总有效率高于对照组(P<0.05)。结论中医正骨联合理筋手法治疗儿童寰枢关节半脱位疗效确切,可明显促进患儿症状、体征的好转,提高临床疗效,操作简单方便。Objective To observe the clinical effect of traditional Chinese medicine(TCM)bone-setting combined with tendon-soothing maneuver for atlantoaxial subluxation(AAS)in children.Methods A total of 64 children with AAS were randomly assigned into the treatment group(n=32)and the control group(n=32).All patients were managed by tendon-soothing maneuver at the neck,and those in the treatment group were additionally treated with TCM bone-setting.A 3-week treatment was performed to compare atlanto-dental interval(ADI),anterior atlanto-dental interval(AADI).The curative effect was assessed.Results After treatment,ADI and AADI in the both groups were decreased(all P<0.05),which were decreased in the treatment group compared with the control group(all P<0.05).The overall effective rate in the treatment group was better than that in the control group(97%[31/32]vs 75%[24/32],[P<0.05],respectively).Conclusion For children with AAS,TCM bone-setting combined with tendon-soothing maneuver has definite curative effect,which can significantly promote the improvement of children's symptoms and signs,improve clinical curative effect,with simplicity and convenience.
