清热凉血汤联合308准分子激光治疗血虚风热型白癜风的临床研究

摘　　要：目的观察清热凉血汤联合308准分子激光治疗血虚风热型白癜风的临床疗效。方法将90例血虚风热型白癜风患者按照随机数字表法分为2组,对照组45例予308准分子激光治疗,治疗组45例在对照组基础上加用清热凉血汤治疗。2组均治疗8周后统计疗效,比较2组治疗前后中医症状(包括局部白斑、面色淡白、食欲不振、头晕眼花、心悸失眠、乏力)评分、汉密尔顿抑郁评定量表(HAMD)评分变化情况,观察2组治疗期间不良反应发生情况。结果治疗组总有效率93.33%(42/45),不良反应总发生率13.33%(6/45),对照组分别为77.78%(35/45)、8.89%(4/45),治疗组疗效优于对照组(P<0.05),但2组不良反应总发生率比较差异无统计学意义(P>0.05)。与本组治疗前比较,2组治疗后中医症状局部白斑、面色淡白、食欲不振、头晕眼花、心悸失眠、乏力评分均降低(P<0.05),且治疗组治疗后局部白斑、面色淡白、食欲不振、头晕眼花、心悸失眠、乏力评分均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后HAMD评分均降低(P<0.05),且治疗组治疗后HAMD评分低于对照组(P<0.05)。结论清热凉血汤联合308准分子激光治疗血虚风热型白癜风临床疗效确切,可效改善患者中医症状,缓解抑郁情绪,且安全性好。Objective To observe the clinical efficacy of Qingre Liangxue Decoction combined with 308 excimer laser for vitiligo of blood deficiency and wind-heat type.Methods A total of 90 patients with vitiligo(blood deficiency and wind-heat type)were randomly assigned into the treatment group(n=45)and the control group(n=45).All patients were managed by 308 excimer laser,and those in the treatment group were additionally treated with Qingre Liangxue Decoction.Treated for 8 weeks,the aim was to compare traditional Chinese medicine(TCM)symptom(local white spots,pale complexion,loss of appetite,dizziness,palpitations,insomnia,fatigue)score,Hamilton Depression Rating Scale(HAMD).The curative effect and adverse reactions were assessed.Results The overall effective rate was higher in the treatment group(93.33[42/45])compared with the control group(77.78%[35/45]),(P<0.05).The difference was not statistically significant in the total incidence of adverse reactions between the treatment group and the control group(13.33%[6/45]vs 8.89%[4/45],[P>0.05],respectively).After treatment,TCM symptom scores in groups were decreased(P<0.05),which were decreased in the treatment group in comparison with the control group(P<0.05).HAMD scores in groups were decreased(P<0.05),the reduction in the treatment group was more common compared with the control group(P<0.05).Conclusion For patients with vitiligo(blood deficiency and wind-heat type),Qingre Liangxue Decoction combined with 308 excimer laser has definite clinical curative effect,which can effectively improve TCM symptoms,relieve depression,and with good safety.
