阿是穴火针联合CT引导下背根神经节热凝射频术治疗带状疱疹后神经痛的临床疗效

摘　　要：目的观察阿是穴火针联合CT引导下背根神经节热凝射频术治疗带状疱疹后神经痛(PHN)的临床疗效。方法将48例PHN患者按照随机数字表法分为2组,对照组24例予CT引导下背根神经节热凝射频术治疗,治疗组24例在对照组基础上加用阿是穴火针治疗。2组均治疗后4周评定疗效,比较2组治疗前及治疗后1 d、4周疼痛视觉模拟评分(VAS)、抑郁自评量表(SDS)评分及匹兹堡睡眠质量指数(PSQI)评分变化情况。结果治疗组愈显率75.0%(18/24),总有效率95.8%(23/24),对照组分别为33.3%(8/24)、83.3%(20/24),治疗组愈显率及总有效率均高于对照组(P<0.05)。与本组治疗前比较,2组治疗1 d、4周疼痛VAS均降低(P<0.05),且治疗组治疗1 d、4周疼痛VAS评分均低于对照组同期(P<0.05)。与本组治疗前比较,2组治疗1 d、4周SDS评分均降低(P<0.05),且治疗组治疗4周SDS评分低于对照组同期(P<0.05)。与本组治疗前比较,2组治疗1 d、4周PSQI评分均降低(P<0.05),且治疗组治疗1 d、4周PSQI评分低于对照组同期(P<0.05)。结论阿是穴火针联合CT引导下背根神经节射频热凝术治疗PHN疗效确切,可快速缓解患者疼痛症状,改善睡眠障碍,缓解抑郁情绪,提高生活质量,且安全性高,疗效稳定。Objective To observe the curative effect of fire needle at Ashi point combined with CT-guided dorsal root ganglion radiofrequency thermocoagulation for postherpetic neuralgia(PHN).Methods A total of 48 PHN patients were randomly assigned into the treatment group(n=24)and the control group(n=24).All patients were managed by CT-guided dorsal root ganglion radiofrequency thermocoagulation,and those in the treatment group were additionally treated with fire needle at Ashi point.Treated for 4 weeks,the aim was to compare visual analogue scale(VAS)for pain,Self-rating Depression Scale(SDS)scores,Pittsburgh Sleep Quality Index(PSQI)scores on days 0,1 and on week 4;the curative effect was assessed.Results The cured and markedly effective rate and the overall effective rate in the treatment group were better than those in the control group(75.0%[18/24]vs 33.3%[8/24],95.8%[23/24]vs 83.3%[20/24],[P<0.05],respectively).On day 1 and on week 4,VAS for pain and PSQI scores in groups were decreased(all P<0.05),which were decreased in the treatment group compared with the concurrent control group(all P<0.05).SDS scores on day 1 and on week 4 in groups were decreased(P<0.05),SDS scores on week 4 in the treatment group were lower than those in the concurrent control group(P<0.05).Conclusion For PHN patients,fire needle at Ashi point combined with CT-guided dorsal root ganglion radiofrequency thermocoagulation has definite curative effect,can quickly relieve pain symptoms,improve sleep disorders,relieve depression,improve quality of life,and with high safety and stable curative effect.
