帕金森病异动症的中西医临床治疗研究概况

摘　　要：现代医学多认为帕金森病异动症(LID)主要与左旋多巴药物代谢、间歇多巴胺刺激、脑解剖组织黑质纹状体以及皮质-基底节-丘脑-皮质环路中的多种改变有关;危险因素包括发病年龄、左旋多巴等效剂量、性别、遗传变异等;临床以调整左旋多巴服药次数、剂量或增加药物,以及手术治疗常见。中医认为本病基本病机为肝风内动,病位在筋脉,风、痰、瘀、毒互结壅滞,病理性质总属本虚标实,治疗应以补虚、息风、活血、化痰为主,另外针灸及康复治疗可有效改善LID症状、减轻西药治疗带来的不良反应。参考文献37篇。Modern medicine believes that levodopa-induced dyskinesia(LID) is mainly related to the drug metabolism of levodopa,intermittent dopamine stimulation,various changes in the substantia nigra striatum of brain anatomy and the cortex-basal ganglia-thalamus-cortex loop. Risk factors include onset age,levodopa equivalent dose,gender,genetic variation,etc. It is common for clinical treatment to adjust the times and dosage of levodopa or add drugs,as well as for surgical treatment. Traditional Chinese medicine believes that the basic pathogenesis of this disease is internal stirring of liver wind,the disease location is in the muscles and tendons,wind,phlegm,blood stasis and toxin are intertwined and stagnated,and the pathological nature is always root deficiency and branch excess. The treatment should focus on tonifying deficiency,extinguishing wind,activating blood and resolving phlegm. In addition,acupuncture,moxibustion and rehabilitation treatment can effectively improve the symptoms of LID and reduce the adverse reactions caused by western medicine treatment.
