摘　　要：目的:研究不稳定性肱骨髁上骨折患儿经闭合复位经皮克氏针内固定的疗效及护理。方法:回顾性分析83例不稳定性肱骨髁上骨折患儿的病历资料。所有受试者均接受闭合复位经皮克氏针内固定治疗,将其按照临床护理方式的差异分为观察组(n=41)与对照组(n=42)。对照组开展常规护理干预,观察组则予以综合护理干预。分析两组临床疗效、住院时长、骨折愈合时长、医疗费用、术后并发症发生情况及干预前后心理弹性变化情况。结果:观察组优良率高于常规组,差异有统计学意义(P<0.05)。观察组住院时长及骨折愈合时长明显短于常规组,差异有统计学意义(P<0.05)。两组术后骨折再转移、切口感染以及骨化性肌炎发生率对比差异均无统计学意义(P>0.05)。观察组干预后各项CD-RISC评分均高于对照组,差异有统计学意义(P<0.05)。结论:综合护理应用于闭合复位经皮克氏针内固定治疗不稳定性肱骨髁上骨折患儿的疗效显著,可促进患儿骨折愈合,改善患儿心理弹性水平,值得临床推广应用。Objective To study the curative effect and clinical nursing measures of closed reduction and transpicker wire internal fixation in children with unstable humerus supracondylar fracture.Method Medical records of 83 children with unstable supracondylar fractures of the humerus were retrospectively analyzed.All subjects received closed reduction and picker’s wire fixation, and were divided into observation group(n=41) and control group(n=42) according to the difference of clinical nursing methods.Routine nursing intervention was carried out in the control group and comprehensive nursing intervention was carried out in the observation group.The clinical efficacy, length of hospital stay, length of fracture healing and medical costs, postoperative complications and changes in psychological resilience before and after intervention were analyzed.Results The excellent and good rate of observation group was higher than that of conventional group(P<0.05).The length of hospitalization and fracture healing in the observation group were significantly lower than those in the conventional group(P<0.05).There were no significant differences in the incidence of postoperative fracture remetastasis, incision infection and myositis ossificans between the two groups(P>0.05).CD-RISC scores in observation group were higher than those in control group after intervention(P<0.05).Conclusion Comprehensive nursing applied in the treatment of children with unstable supracondylar fracture of humerus by closed reduction and internal fixation with Picker’s needle has a significant effect, which can promote fracture healing and improve the level of psychological resilience of children, and is worthy of clinical application.
