综合护理对ICU经口气管插管患者机械通气期间呼吸机相关性肺炎的影响

摘　　要：目的 探讨研究在ICU经口气管插管患者的机械通气期间对其实施综合护理,对患者呼吸机相关肺炎的影响效果。方法 研究样本选取时间为2019年5月至2021年5月笔者所在医院ICU实施经口气管插管机械通气患者,共计120例,分组方式为随机抽签,分为试验组与对照组,两组患者例数均为60例。对照组患者实施常规临床护理干预,试验组则是予以患者综合护理干预,比较两组患者的呼吸机相关性肺炎发生率及干预前后的血气、心理指标。结果 试验组患者呼吸机相关性肺炎发生率显著低于对照组,差异显著(P <0.05);且在干预后试验组患者的血氧分压、动脉二氧化碳分压、血氧饱和度及汉密尔顿焦虑量表指标均优于对照组,差异显著(P <0.05)。结论 在ICU经口气管插管呼吸支持患者的护理中实施综合护理具备理想的应用效果,可显著降低患者的呼吸机相关肺炎发生率,值得临床推广。Objective To investigate the effect of implementing comprehensive care to patients with ventilator-associated pneumonia during mechanical ventilation of orotracheal intubated patients in the ICU. Methods The study sample selection period was from May 2019 to May 2021. A total of 120patients were enrolled in the implementation of mechanical ventilation with orotracheal intubation in the ICU of the author’s hospital, and the grouping mode was random draw, the group name was experimental group versus reference group, and the number of patients in both groups was 60 cases. Patients in the reference group were implementing routine clinical nursing intervention, while patients in the experimental group were receiving comprehensive nursing intervention to compare the incidence of ventilator-associated pneumonia between the two groups and the blood gases and psychological indexes before and after the intervention. Results The experimental group suffered significantly less ventilator-associated pneumonia than the reference group(P<0.05). Moreover, the partial pressure of blood oxygen, partial pressure of arterial carbon dioxide, partial pressure of oxygen saturation and Hamilton Anxiety Scale indexes of the patients in the experimental group after the intervention were better for the reference group, with significant differences(P<0.05). Conclusions The implementation of integrated care in the care of patients with oral tracheal intubation respiratory support in the ICU has desirable application results, can significantly reduce the incidence of ventilator-associated pneumonia in patients, and is worthy of clinical promotion.
