摘 要:目的评价CO_(2)点阵激光联合浓缩纳米脂肪移植治疗面部萎缩性瘢痕的临床疗效和安全性。方法回顾性分析2017年6月至2019年5月,烧伤、外伤后面部萎缩性瘢痕患者40例的临床资料,分为两组,每组患者20例。两组均采用超脉冲CO_(2)点阵激光治疗,能量密度50~60mJ/cm^(2),光斑密度10%,平扫1遍,共治疗2次,每次治疗间隔2个月。对照组采用点阵激光治疗后不行其他治疗;观察组第2次点阵激光治疗后1周进行浓缩纳米脂肪移植治疗。两组患者治疗后6个月随访,采用温哥华瘢痕量表(vancouver scar scale,VSS)进行评分,判定临床疗效,并记录并发症和不良反应发生情况。结果治疗后6个月,两组患者VSS评分较治疗前均有下降,差异具有统计学意义(P<0.05);观察组VSS评分显著低于对照组(P<0.05)。治疗后6个月,观察组总有效率高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论CO_(2)点阵激光联合纳米脂肪移植可有效治疗萎缩性瘢痕,安全性好,值得临床推广应用。Objective To compare and evaluate the efficacy and safety of fractional carbon dioxide laser combined with the condensed nanofat grafts for the facial atrophic scars.Method A retrospective study was made on 40 patients with facial atrophic scars treated over the period from Jun.2017 to May 2019.They were divided into two groups,the control group,of which the 20 patients were given only fractional CO_(2)laser treatment twice at an interval of 2 months,and the observation group,of which the 20 patients were given condensed nanofat grafts one week after the second laser treatment.Vancouver scar scale(VSS)scoring and clinical efficacy valuation were performed based on the patients’photos before and 6 months after the treatments,the adverse reactions were recorded,and a blind follow-up was carried out by two researchers.Results After the treatment,the VSS scores dropped remarkably in both groups(P<0.05),and the decrease was much greater in the observation group than in the control group(P<0.05).Moreover,the therapeutic effect of the observation group was better than that of the control group(P<0.05).Conclusions For facial atrophic scars,both fractional laser and nanofat grafts are effective,but the curative effect of the combination of the two is better than that of simple fractional laser treatment.
